Integrated Access and Flow Navigator

Alexandra Hospital, Ingersoll and Tillsonburg District Memorial Hospital are actively seeking applicants for an Integrated Access and Flow Navigator position. "Integrated" means that the successful incumbent will work at both hospital sites.

The Access and Flow Navigator will physically work between both sites, in accordance with established standards, professional requirements and as a member of the multidisciplinary health care team the Patient Care Coordinator provides administrative and clinical support to all staff in a supportive, empowering manner while facilitating the effective and efficient movement of admitted patients through the hospital system. Responsible for hour-by-hour planning, organizing, directing, coordinating, and evaluating activities related to bed utilization and patient movement that includes bed assignments, admission and discharge coordination, patient discharge and repatriations.
**Key Responsibilities**:
- Responsible for patient flow at TDMH and AHI in collaboration with the ED Charge Nurse, Inpatient Charge Nurse and the Clinical Leader
- Coordinate daily operations of patient flow
- Coordinate repatriations and inter-facility transfer requests, including updating tracking spreadsheet and Critical
- Collaborate with physicians, allied health professionals, home and community care, patients and families to help with patient discharge planning
- Coordinate with Charge Nurses/Clinical Leaders regarding bed allocation for admissions
- Attend patient family meetings as needed
- Attend patient care rounds
- Attend and lead huddles in absence of Clinical Manager
- Assist in planning, coordinating, and implementing of quality improvement initiatives
- Collaborate with home and community care and social worker to navigate ALC patients
- Collaborate with patients/families regarding ALC or chronic care co-payment
- Work collaboratively with the ward clerk and assist with any patient related concerns
- Participate in implementing and coordinating a patient-centered care plan with patients, family members, significant others, and the multi-disciplinary team for the purpose of facilitating the patient's movement through the continuum of care
- Provide a link between the hospital community agencies and alternate level of care facilities.
- Liaison and establish contact with members of the multi-disciplinary team and appropriate community agencies to obtain, provide or exchange information relating to patients' psycho-social functioning and health care in the past, present and future, in conjunction with the clinical team.
- Educate hospital staff to the various aspects of patient access and flow, discharge planning and available resources.
- Acting as a clinical resource for the acute inter-professional teams by providing information based on clinical knowledge and theory/practice related to clients with complex health care needs.
- Coordinate the daily bed requirements of patients being admitted/transferred within the hospital.
- Establish and maintain a systematic review and assessment of bed availability, and surge planning.
- Ensure appropriate patient assignment based on clinical need, working with other departments to assess the need for standards to optimize the utilization of beds (inclusive of pre-operative days and direct admissions).
**Qualifications**:
- Member of a professional college and in good standing
- Five (5) years practical and related clinical experience.
- Knowledge and experience in access & flow, discharge planning and case management
- Proof of Basic Life Support for Health Care Providers
- Experience with Microsoft Office
- Experience with Cerner
- Demonstrates excellent verbal and written communication skills
- Strong interpersonal communication skills and customer service experience
- Demonstrated ability to coach teams effectively.
- Demonstrated interest in continuing professional growth for self and effectively encourages other towards this goal.
- Demonstrated critical thinking and excellent problem solving skills
- Ability to develop positive working relationships within a team to contribute to a positive work life.
- Is cognizant of emotions and experiences of others while communicating with patients, families and team members.
