Job Description:
Position Overview:
The Psychogeriatric Resource Consultation Program (PRCP) in the Greater Toronto Area (GTA) provides knowledge-to-practice (KTP) services to staff in long-term care homes, community services agencies and home and community care organizations, who support older adults living with cognitive changes and/or mental health conditions and who may be experiencing challenging behaviours. This may also include vulnerable adults who may be less than 65 years of age but whose cognitive and functional abilities are compromised by early onset dementia, developmental disability or acquired brain injury.

The Psychogeriatric Resource Consultant (PRC) will be an employee of Scarborough Health Network (SHN) and liaise with appropriate ‘in-house’ services and local psychogeriatric outreach and Behavioural Supports Ontario services. As well, the consultant will join a team of 9 other PRCs distributed across the region and report to the PRC Program in a distributed matrix reporting model.

While based at SHN, the consultant’s work will be focused in long-term care homes, community service agencies and home and community care organizations in an assigned catchment area. Activities will include participation in in-services, behaviour support rounds/huddles, case-based discussion and coaching, as well we delivery of core curriculum.

The PRC will be required to provide services in accordance with the PRCP’s KTP framework leading to practice and workplace improvements within the service organizations and enhance service integration. As well the PRC will optimize opportunities to collaborate with other PRCs, demonstrate leadership in the care of the target population and participate in the program’s accountability initiatives. Each PRC is responsible to adjust her/his work schedule to meet the needs of their client organizations.

**Requirements**:

- A degree and/or certification in one of the following disciplines - nursing, social work, rehabilitation (OT /PT), psychology or gerontology
- At least two years experience in psychogeriatic services is preferred, otherwise significant experience in geriatrics, long-term or community-based seniors care is essential
- Demonstrated competence with standard psychogeriatric assessment tools
- Knowledge and experience in adult education
- Driver’s license and the ability to travel across the region and the GTA on a daily basis
- Demonstrates a commitment to customer service when dealing with patients, colleagues and volunteers and other members of the community and public
- Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
- Demonstrates an understanding of and commitment to SHN's comprehensive safety programs and practices including staff safety, patient safety, and environmental safety and participates in regular in-service education in this area
- Demonstrates excellent work performance and attendance record
