We are currently looking to hire a:
**Intensive Case Manager**
**3 - Full-Time, 6-Month Contract Positions (35 hours per week)**
**Reporting To**:Manager, Community Services
The Intensive Case Manger works in a community-based health center setting and is responsible for assessing, treating, and evaluating individual, interpersonal and societal problems through the use of their social service work knowledge, skills, interventions and strategies in order to assist individuals, families, groups, organizations and communities to achieve optimum social functioning.
**Purpose**:
- To be a contributing member of the program in delivering client-focused care/service and life skills workshops based on best-practice principles.
- To be an advocate for the client by assessing clients' needs or eligibility for specific services and to follow through with recommendations.
- To conduct follow-up assessments.
- To resolve conflict situations.
- To be an integral member of the program in delivering funder’s expectations.
- To liaise for clients and others by providing relevant information about other SCHC services.
**Key Areas of Responsibility**:
- Conduct intake, assessment and case management services, utilizing a client-centered and strength-based approach.
- Report and record client status after each visit on EMR system (PS Suites).
- Knowledge and understanding of ODSP/OW.
- Work collaboratively and communicate effectively with clients, co-staff, management and other providers.
- Coach or assist a client in the development/maintenance of abilities relating to activities of daily living, self-care, crisis management strategies, symptom management, maintaining a support network, and skills teaching such as budgeting, grocery shopping and other life skills.
- Deliver services efficiently within scope of practice.
- Engage and participate in scheduled Scarborough Centre for Healthy Communities events/activities.
- Perform other duties as assigned that are reasonable within the scope of the job.

- Flexibility and ability to work all shifts.
- Strong organizational, interpersonal, and communication skills.
- Willingness to work at multiple site/locations.
- Good assessment and critical thinking skills.
- Sensitivity and awareness of cultural, racial, economic, and socially diverse communities.
- Oral and/or written fluency in other languages an asset.
- Driver’s license and access to vehicle is an asset.
- Second language is considered an asset.
- Current Standard and/or Emergency First Aid and CPR certification.
- Vulnerable Sector check and clearance.
**Educational and/or Professional Qualifications**:
- Social Services Worker Certificate from a recognized and accredited school.
- Member of a professional regulatory body is considered an asset.
**Level of Experience**:
- Two to three years of experience in a community setting is an asset.
**Significant Working Conditions**:
- Ability to travel between clients in community.
- Ability to deal with clients in a professional and courteous manner.
- Flexibility of hours - Required to work different shifts (day, evening, overnight and weekend shift).
- Working environment may include common spaces or residents homes which may not be smoke-free, may have pets or pests.
**Remuneration**:$27.96 - $34.27
