**Main Purpose of the Job**:
Under the direction of the Social Supports Manager, the CEE Social Support Worker (SSW) delivers black youth focused, culturally relevant wraparound support as part of Social Support Team.
The duties may vary depending on psycho-social needs of each youth, from providing one-to-one case management support with each youth to facilitating mental health and wellness workshops. By assisting each youth that may be affected by a wide range of challenges, such as trauma, mental health, abuse, domestic violence, housing, financial constraints, etc., the SSW ensures each youth has the safe space where they feel empowered, build their resilience and become emotionally independent. The SSW will be in charge of assessing each youth’s assets and needs, providing system navigation case management, facilitating one-to-one coaching/supportive counseling sessions, liaising with internal and external service providers, and delivering educational workshops.
A key to succeeding in this role is the ability to recognize each youth’s strengths and unique needs from a holistic perspective. Ability to intervene to support youth experiencing a crisis is a must, combining with empathy-based solution focused approach while maintaining professional boundaries.
As part of CEE’s Programs Department, the SSW reports to the Manager of Social Support Team, and will closely work with CEE’s Program Delivery Team, ensuring that each youth receives timely, quality services.
As a rapidly growing B3 (Black-focused, Black-led, and Black-serving group) organization, CEE Centre for Young Black Professionals plays a critical role in lifting up each black youth by providing opportunities and resources they deserve. Recent graduates of SSW or BSW with direct work experience providing black youth focused case management and supportive counseling/coaching are highly encouraged to pursue this growth opportunity

**Duties and Responsibilities**:
Primary
- Attend program retreats (sometimes overnight) and be present in-class/online to build rapport with youth participating in CEE’s programs.
- Coordinate social support services and referrals between CEE and community agencies.
- Provide general counseling/social services to current program participants and alumni as assigned.
- Delivery of assigned workshops in-class (virtual or in-person).
- Maintaining accurate records, case notes and preparing reports for internal and external stakeholders.
- Provide crisis management support to youth in consultation with the Social Support Manager and CEE team. Responsible for delivering services in ways that minimize crisis escalation and victimization.
- Contribute to refining CEE’s programs/services around the promotion of healthy lifestyles and the management of crisis situations
- Support development/redevelopment of intake form, participant needs assessment, workshops, progress and outcome documents.
- Other duties and tasks as assigned by the Manager of Social Supports Team

**Qualifications**:

- Possess an understanding of the impacts of trauma, anti-Black racism and social determinants of health and how they manifest in Black youth
- Cultural competence working in diverse Black communities, demonstrated ability to articulate the issues, needs, and realities of black youth in Toronto’s priority neighborhoods
- Ability to maintain professional boundaries while building authentic relationships with youth
- Experience working with youth facing mental health challenges
- Significant experience managing risk, exercising judgment and supporting clients and co-workers in crisis situations in a complex, high needs environment
- Experience gathering quantitative and qualitative data for reporting purposes
- Training and experience in conflict resolution considered an asset
- Establish referral networks/linkages with outside agencies for clients and their families when appropriate.
- Track each youth’s progress within the program and follow up on all the referrals made.
- Create workshop content for assigned workshops.
- Valid driver’s license and access to own vehicle considered an asset
- Registration with OCSWSSW in good standing considered an asset
