**PRIMARY FUNCTIONS**:
Leads regional stakeholders in the planning, implementation and evaluation of a regional stroke work plan which addresses health system change and continuous improvement in stroke prevention, care, recovery, and community reintegration.

Specifically, this role facilitates implementation and sustainability of patient-centred, stroke best practices locally, regionally and provincially, across the care continuum with a focus on
**prevention, hyperacute and acute car**e. This involves continuously providing clinical and professional leadership, raising awareness, providing education, informing models of care delivery, building capacity in others/organizations, managing projects, monitoring data and conducting evaluations. Project examples include engaging multiple stakeholders in health system change and in developing or implementing best practice tools and resources.

**DUTIES & RESPONSIBILITIES**:
Lead and enable health care providers including administrators and health care organizations across the continuum to identify, plan, and implement strategies to enhance and sustain best practice stroke prevention and care. Strategies may include but are not limited to:
- Program development and development of resources (e.g. care paths and algorithms, protocols and guidelines) designed to standardize and improve care and patient/family experience;-
- Mentorship and consultation;-
- Provision of formal and informal education;-
- Linking providers & administrators to appropriate tools & resourcesParticipate in evaluation design, monitoring, analysis and change management. This includes program monitoring and evaluation to identify best practice gaps including inefficiencies; planning and monitoring continuous improvements in practice. This includes the implementation of Accreditation Canada’s Stroke Distinction program.

Advise on new models of care delivery including staffing models. Enable/support change including health care re-design. Assist and advise on proposals and business cases that will address care gaps. Investigate innovative approaches or programs to improve health outcomes including system integration opportunities for better efficiencies. Inform strategies to sustain and improve performance.

Support the ongoing enhancement of interprofessional collaboration amongst and across provider teams and agencies (interprofessional communication tools, conflict management, facilitation, reflection, collaborative decision making) (e.g., ED, IVR, Critical Care and Acute Stroke Teams in the development of Endovascular Stroke Program). Work collaboratively with teams in the development and monitoring of regional and local work plans designed to improve care. Use innovative methods to communicate best practices.

**BASIC QUALIFICATIONS**:
- Undergraduate or graduate degree in a Health Profession (entry to practice)-
- Masters in a health-related field (e.g. health leadership, project management, change management, quality, health research, business, health education/rehabilitation/nursing, health policy, public administration etc.)-
- 10 years health care experience including a minimum of 2 years of acute care experience-
- 2 years’ experience in health care leadership or change management (e.g.; program management, program evaluation, project management, change management, continuous quality improvement, knowledge translation)-
- Clinical experience in delivery of stroke care with demonstrated knowledge of stroke care continuum.-
- Clinical experience working with an interprofessional team-
- Previous experience as a change agent with an understanding of organizational cultures-
- Demonstrated leadership, interpersonal and conflict resolution skills with the ability to work both collaboratively and independently.-
- Experience in a variety of clinical settings including acute and emergency care (preferred)-
- Licensed member in good standing with a health professional college and/or member of the Regulated Health Professionals of Ontario-
- Strong consultative skills; strong oral and written communication skills-
- Computer literate, experience with databases-
- Valid driver’s license and own transportation-
