This regional role directs/leads the Stroke Network of Southeastern Ontario in delivering health system change, professional education, and knowledge translation to continuously improve access to best practice quality care across the stroke care continuum.
The role is accountable for change management: planning, implementation, evaluation and continuous improvement of a regional integrated system of stroke care. The role directs program operations and engages the regional stroke team and regional stakeholders in identifying priorities for change, developing, delivering, and monitoring a Regional Stroke Workplan.
Strategic alliances are created with a broad range of stakeholders including consumers, health care providers, community organizations, government agencies (Ontario Health, CorHealth Ontario), education institutions (Queen’s University, St Lawrence College), the Heart and Stroke Foundation. The role also represents the regional program and advises on the system of stroke care at a provincial and national level.
This role is accountable for contributing to the delivery of the KHSC’s strategy. The role must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do.
**RESPONSIBILITIES & DUTIES INCLUDE**:
**Regional Planning & Change Management for Best Practice Patient Care**
Per Ministry of Health Service Guidelines for Regional Stroke Centres, the Regional Director is accountable for the delivery of a Regional Stroke Workplan that addresses planning, implementation, monitoring/evaluation, and continuous quality improvement of an integrated System of Stroke Care across the continuum of care and across Southeastern Ontario. Leads teams in health system change and professional education/knowledge translation to improve access to Best Practices across the Stroke Care continuum. Establishes organized models of stroke care delivery within the region, including new stroke care staffing models. Leads a team in coordinated project management, managing many projects in differing sectors and regional locations at once. Also leads the team in event management, delivery of regional education, conferences, workshops and knowledge translation programs. This role monitors stroke performance and is accountable for achieving established provincial performance indicators to improve patient outcomes in stroke prevention and care delivery.
**Strategic Alliances - Coordination and Communications**
Engages all stakeholders in setting priorities and delivering on the Regional Stroke Plan. Supports the Regional Stroke Centre accountability to work collaboratively with a Regional Stroke Steering Committee in oversight and delivery of the Plan outlined above. Manages regional strategic alliances with all stakeholders including care consumers, healthcare providers working across the care continuum, government agencies, education institutions, the Heart and Stroke Foundation and Accreditation Canada. Health care partners include 12 hospital sites in the region (acute and rehab), 5 paramedic services, primary care teams, ambulatory stroke clinics, emergency departments, acute and rehabilitation stroke units, community agencies and 40 Long Term Care homes. Ensures effective communication strategies are in place to support engagement and collaboration in an organized system of care delivery. Represents the regional program and advises on the system of stroke care at a provincial and national level.
