Home and Community Care Support Services South East is one of 14 Home and Community Care Support Services organizations in Ontario with a focused mandate to deliver local health care services such as home and community care and long-term care home placement.

These organizations were previously known as Local Health Integration Networks (or "LHINs") at a time when they also led local health care planning and funding, and those functions are now part of Ontario Health.

Home and Community Care Support Services South East is dedicated to ensuring the ongoing delivery of local services while Ontario makes changes to improve the health care system to give patients better-connected care with health care providers working as one coordinated team in Ontario Health Teams.
The south east region extends from Brighton on the west, to Prescott and Cardinal on the east, north to Perth and Smiths Falls, and back to Bancroft. The boundaries are for funding and planning purposes only. Residents of the Home and Community Care Support Services South East can seek health care services wherever they prefer, inside or outside these boundaries.

As a Direct Care Facilitator, you will coordinate Medical Assistance in Dying (MAiD) requests, by facilitating referrals for our Direct Care Nursing team. You will also review and manage direct care referral waitlists, and facilitate the submission of patient care documents to external partners as required for patient flow. You will act as an information resource to internal and external stakeholders.

**What will you do?**
- Receive and monitor referrals for Direct Care nursing within the HCCSS South East geography through our CHRIS system
- Ensure that information regarding waitlists is kept current on the data base for Direct Care Nursing programs
- Contact patients/families/primary care partners as required for initiation of referrals and services for MAiD
- Coordinate referrals for MAiD, TSN, RRN and related programs
- Complete and organize documents and submissions from Nurse Practitioners that are submitted to external partners
- Document admissions according to policies and procedures
- Liaise with Direct Care Nurses, Team Assistants, Care Coordinators, and other internal staff to resolve crisis situations
- Prepares and maintains statistics of referrals and admissions to Direct Care Nursing programs in HCCSS South East.

What must you have?
- Current registration with the College of Nurses of Ontario as an Registered Practical Nurse (RPN)in the general class
- Health Care experience in medical administration, or equivalent HCCSS South East experience is preferred
- Knowledge of PHIPA, Substitute Decision Act, and Health Care Consent Act
- Strong data entry skills and ability to work with MS Word, Outlook, Excel and the Internet
- Knowledge of the health care delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
- Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment
- Established ability to accurately complete required documentation, reports and forms
- A valid driver’s licence and access to a reliable vehicle
- Proficient in a Windows environment
