**HOURS: WEEKDAYS & SOME EVENINGS - 0800 HOURS TO 1600 HOURS
The Patient/Client Flow Navigator will support the philosophy that each patient receives the right level of care in the right place
at the right time by the right provider. Responsibilities of this position include but are not limited to:

Promoting optimal patient and clinical outcomes by assessing and facilitating the effective management of care,
utilization of resources and length of stay and waiting time to services

Addressing issues impeding the patient’s continuity of care in the community

Coordinating the multidisciplinary team‘s plan of care to make sure needs are met in a timely manner and consistent
messaging to the patient and family regarding the continuity of care plans is ensured

Coaching patients to become advocates for their own care

Endorsing inter-professional collaboration related to length of stay

Working with members of the interdisciplinary team, psychiatrists and community partners in support of effective
professional and collaborative practice. Contributing to the planning, implementing and evaluating of mental health
treatment services in a hospital setting or in the community

Identifying and addressing gaps in clinical practice as well as facilitating addressing of system level gaps and risk
management issues

**QUALIFICATIONS**: 
Current Certificate of Registration ~ College of Nurses of Ontario required 
Minimum of five (5) years of recent acute care experience required 
Extensive knowledge gained through in-depth involvement in a complex technical/professional field and/or broad experience

in several fields required 

Demonstrated ability to work collaboratively in a multidisciplinary team environment including internal and external partners
required 

Demonstrated strong leadership ability required 
Demonstrated facilitation, critical thinking and problem solving skills required 
Demonstrated exceptional organizational skills required 
Data collection and analysis/accuracy experience required 
Ability to rapidly shift workload priorities 
Ability to provide leadership in emergency situations 
Demonstrated ability to be self-directed and thrive in a dynamic environment 
Ability to support continuous quality improvement initiatives 
Demonstrated knowledge of the various mental health services within the NBRHC and in North Bay 
Good attendance and work record required 
Must demonstrate commitment to ensure your safety, the safety of fellow workers, patients, volunteers and visitors 
Must support and contribute to a culture of safety and prevention of adverse health events in this organization 
