**POSTION SUMMARY**:
Under the direction of the Administrator, is responsible for day-to-day operation of the Community Support Services Programs - Supportive Housing, Caregiver Support/Respite, Home Help/Homemaking, and Home Maintenance.

**ESSENTIAL DUTIES AND RESPONSIBILITIES**:
Undertake the following key responsibilities as they pertain to the programs and services provided by the agency including but not confined to:

- Assessment and eligibility determination;
- Service plan development;
- Review and evaluation and revision of the client’s plan of service;
- Service co-ordination and ensuring the provision of services;
- Waiting list management (if applicable);
- Ensure that there are client safeguards and provisions for client involvement in the assessment process and development of service plans;
- Ensure that clients are advised of their rights and the complaint process established by the Cassellholme Community Support Services_**; **_
- Participates in the planning and development of the Quality Improvement Program;
Provide support to the client and their caregiver to:

- Assist and support caregivers who must act on behalf of persons who are no longer capable regarding their personal care;
- Assist clients to access and use other services in the community;
- Develop strategies for clients living at risk in the community who for any number of reasons may be resistant to service_**; **_
- Provide information to the client regarding services provided by Cassellholme Community Support Services;
- Refer clients to appropriate services (eg. other service providers, other agencies) if necessary;
Work collaboratively with other service providers for the following purposes:

- Developing collaborative service plans for clients with multiple needs;
- May participate in the development of area wide service delivery strategies for clients who are require community support services vulnerable, or are at risk of abuse or being abused, or living at risk in the community.

Administrative Responsibilities:

- Participating in the development of the annual operating budget and reviewing financial statements and taking action on variances to bring expenditures in line with budget.
- Maintaining daily, monthly, quarterly and annual records and statistics and complete all mandatory program reporting requirements
- Managing and coaching staff, performance management and progressive discipline where necessary;
- Bridge management and employee relations by addressing conflicts, grievances or other issues;
- Managing the hiring and orientation of new staff;
- Participates in the development and successful implementation of the department’s annual priorities in alignment with the organizational priorities;
**EDUCATION, TRAINING & EXPERIENCE**
- Degree/diploma in Nursing or Health Sciences;
- RN/RPN with current registration with College of Nurses of Ontario
- Two (2) to three (3) years related experience in community care, preferably in a unionized healthcare environment (or an equivalent combination of education and experience);
- Comprehensive knowledge of ageing, quality improvement, and community care programs;
- Working knowledge of the _Connecting People to Home and Community Care Act, 2020 _and other relevant legislation (e.g. MFIPPA, ESA, OHRC);
- Demonstrated knowledge of budget planning, resource allocation, financial monitoring and reporting;
- Knowledge of tools, systems and databases used in community client care;
- Knowledge of collective agreements/labour relations, and scheduling required;

- Criminal Reference Check including the Vulnerable Sector Check recent within six (6) months is a requirement

**Salary**: $84,339.00-$102,521.00 per year
