**Job Description**:
Home and Community Care Support Services North East offers a wide-range of quality healthcare services and resources to support people of all ages at home, school or in community. In addition to planning, delivering and coordinating care for thousands of people each day in Northeastern Ontario, we also manage eligibility and admissions to long-term care homes, short stay respite, assisted living, and adult day programs. We need caring, motivated people who are driven to help others and make difference in their community, to join our multidisciplinary team.

**Program Lead**

Home and Community Care Support Services North East is seeking a dynamic individual to support program development within the home and community care portfolio. Reporting directly to the Manager, Program Development, this position will lead and implement large initiatives and programs which have been identified as priorities.

**KEY ACCOUNTABILITIES**:

- Keeps informed of initiatives within organization and improvement teams;
- Follows methodology for documenting current state program analysis;
- Develops implementation plans, adheres to timelines, and ensures established goals are achieved;
- Ensures assigned home and community care initiatives are developed in accordance with Home and Community Care Support Services North East policies, relevant legislation, and professional disciplines;
- Monitors and evaluates compliance to business process and training, as well as identifies areas that are opportunities for improvement;
- Consults with stakeholders to gain the required information to move forward with initiatives;
- Represents Home and Community Care Support Services North East on multi-disciplinary committees and community agency working groups.

**POSITION REQUIREMENTS**:

- Post-Secondary degree in a health-care related field, business administration or relevant field;
- Relevant experience working in a community health care environment;
- In-depth knowledge of care coordination services both within Home and Community Care Support Services North East and broader Home & Community Care Support Services, as well as other community resources;
- Ability to assimilate complex concepts and assist with implementing health system improvements and articulate impact on care coordination services;
- Ability to analyze and interpret data and prepare clear, concise and accurate reports which will support effective decision making and priority setting;
- Strong collaboration skills and effective communication skills to coordinate program development and elicit needed information from stakeholders;
- Ability to deliver information effectively in a variety of settings including one-on-one, team meetings and presentations;
- Ability to relate well with others, build relationships and constructively interact with a variety of contacts;
- Knowledge of continuous quality improvement tools and techniques considered and asset, but not required;
- Advanced oral and written proficiency in English is essential.
