**Who is the Campbell River & District Primary Care Network?**
The Campbell River & District Primary Care Network (CRDPCN) plan was approved by the Ministry of Health and is supported by the Family Practice Services Committee (FPSC) and Doctors of BC. The CRDPCN will work towards networking health services, supporting patient medical home development, improving team-based care, and finding primary care providers for unattached patients within the Campbell River & District communities. PCNs are community-based networks of health care professionals and clinics who work with the community to develop local healthcare solutions to meet the unique needs of the community.
**What does PCN Manager do?**
The PCN Manager is responsible for operationalizing the implementation requirements for the British Columbia Ministry of Health’s Primary Care Network Initiative within Campbell River & District. The PCN Manager is hired and employed by the Division of Family Practice (backbone organization), receives strategic direction from the PCN Steering Committee to execute the PCN Service Plan, reports operationally to the Executive Director of Division of Family Practice and oversees the work of other PCN staff.
The PCN Manager engages with the community’s interdisciplinary providers in a collaborative practice model, working towards the achievement of the attributes of the PCN. This includes maintaining strong relationships with the Campbell River & District Division of Family Practice, Health Authority staff, Patient Medical Homes (PMHs), primary care providers (Family Physicians and Nurse Practitioners), local Indigenous communities and Nations, First Nations Health Authority, and community agencies.
**Responsibilities and Duties**
Stakeholder Relations:
- This role is responsible for the effective, collaborative engagement of all community stakeholders. This includes orientating committee members, engaging with First Nations and local indigenous organizations on PCN priorities, and supporting collaborative approaches with various groups and community organizations.
Strategic Leadership:
- This role provides operational leadership for implementing PCN strategies within the Campbell River and district PCN. This includes participation in relevant committees and working groups and ensuring effective communication among stakeholders. The role requires strong leadership in facilitating partnerships and promoting cultural safety and humility to ensure Indigenous partners are active participants in decision-making.
Operational Management:
- This role leads the project management of all PCN strategies, including proposal development, governance structure oversight, operational planning, budget management, quality improvement, and network evaluation. This role ensures that Ministry of Health deliverables for PCN funding are in compliance with reporting requirements.
People Management:
- This role is responsible for overseeing the management of PCN staff and contractors, fostering a cohesive, inclusive, and motivated team. You will manage the placement and orientation of PCN- funded allied health and nursing staff, in collaboration with the Island Health Primary Care Manager, to integrate them into patient medical homes and support in interdisciplinary care implementation.
Financial Management:
- This role assumes responsibility for risk assessment, mitigation and issues management for the PCN. You will work collaboratively with Division staff to ensure financial systems are well-managed, with appropriate controls and regular financial reporting.
**Skills, Competencies & Experience**
- We are seeking a minimum post-secondary degree in a health and/or leadership related discipline with five or more years of collaborative and distributed leadership and management experience, preferably in the healthcare sector.
- A master's degree in health administration and practical community development experience is a strong asset.
- Demonstrated experience in integrated service delivery design, project management, implementation and evaluation with knowledge of the principles of Primary Health care, population health, and health promotion along with the attributes of primary care delivery.
- A demonstrated understanding of Indigenous cultural safety, humility, the TRC Calls to Action related to health, United Nations Declaration on Rights of Indigenous Peoples (UNDRIP), and key policies relevant to the implementation of PCNs in BC.
- Strong interpersonal skills, strategic planning acumen and the ability to lead, plan, manage, implement, organize, prioritize and problem solve in a complex, multi-disciplinary and changing organizational environment.
**Work Environment & Renumeration**
