TEAM LEAD

Responsible to be Team Lead in a Retirement Home setting. The Team Lead PSW is responsible
for the provision of quality, client-centred, personal care to the clients based on the clients’ plan
of care, the principles of customer service, and Bayshore values, policies and procedures.
Functions as a participating member of the multidisciplinary health care team.
- MAJOR RESPONSIBILITIES- Performs personal care and approved delegated tasks to promote the physical, social and

emotional well being of each client.- Respects and promotes the comfort, safety and dignity of all clients.
- Team lead to provide initial contact with new admissions, reviewing current care plan to

see if updates are needed and collaborating with MCP.- Observes and reports significant changes in the client’s physical and emotional condition

- Reports health and safety hazards to responsible MCP
- Performs other delegated duties as assigned.
- LINE OF RESPONSIBILITY- Meet daily or as needed with facility staff to report any client condition updates.
- Update Bayshore Binder with any changes.
- Performs all delegated duties under the direction of the Manager of Clinical Practice

Job Qualification

**Qualifications**:
Education
PSW certificate or equivalent
- Experience
- Minimum of 2 years' experience working in the home health care field.
- Other Skills and Abilities.- Capable of organizing and prioritizing a variety of tasks effectively.
- Supervisory experience would be considered an asset.
- Tansley 2:00 pm - 10:00 pm / every other weekend required.
- Valid driver’s license and reliable vehicle required.
