Emergency preparedness is a critical aspect of CCH operations. It is essential that all staff and physicians receive training and understand their roles and responsibilities in this field.

**Summary of responsibilities**:

Reporting to the Director, Transformation, Project Management Office, and Health Information Services, the Clinical Transformation Manager is responsible for the planning and implementation of process improvement projects within the various clinical departments of the Cornwall Community Hospital. They will work in partnership with physicians, nurses, allied health, health care leaders and other hospital staff to design processes that are Lean, that embody healthcare best practices and that support and uphold the mission, vision and ICARE values of the Cornwall Community Hospital. Projects will help create a safer environment for patients, families, staff, physicians and volunteers, creating a culture of safety recognizing “Patient Safety” as a top priority and promoting CCH as the “Workplace of Choice”.
The position of Clinical Transformation Manager will have full project accountabilities, developing business cases and project charters for the approval of initiatives, then planning and delivering projects following recognized hospital and industry practice. The Manager will provide the primary interface to internal stakeholder departments (CI, HIS, IT, HR, etc) for alignment of resources to deliver project results. They will also liaise with external organizational stakeholders and will consult with the patient community for co-design as a key to project success. They will perform Change Management activities to promote and educate staff, partners, and patients on new clinical processes and best practices.
General Accountabilities:

- Work with hospital staff and leadership and consult performance dashboards to identify potential areas for improvement
- Perform initial analysis to determine current process and root cause of inefficiencies in target areas
- Develop project charters for improvement projects including performance targets or value measures
- Conduct exercises to further map and refine processes to drive out inefficiencies and remove areas of risk
- Develop future state processes in collaboration with all impacted departments and providers
- Test hypothesis and solutions to ensure safety and effectiveness
- Plan for change with appropriate education and communications for impacted people
- Ensure planned activities are coordinate with other hospital initiatives
- Oversee implementation of new/revised processes
- Periodically measure and monitor for success targets to ensure success
- Ensure that operations are self-sustaining and properly transitioned to operations
- Monitor and review current literature for trends and changes in best practices for all areas of hospital operations
- Participate in provincial or regional working groups to contribute to and adopt best practice for Ontario
- Look for opportunities for alignment between CCH and other system partners for practice improvements
- Champion change that will be of benefit to the operations of CCH

**Requirements of the job**:

- Extensive clinical or clinical management experience and in-depth understanding of best practice as it relates to operations in a hospital setting. Minimum of 2 years’ related experience is required
- An undergraduate degree in health administration, nursing, or related field
- Excellent change management and communication skills to lead change activities
- Strong research and analysis skills in the identification and adaptation of best practice
- Demonstrated experience leading process review and improvement exercises with staff members following Lean Healthcare or similar formal methodology
- Proficient in the use and/or configuration of HIS systems (Cerner Millennium preferred) for process support
- Demonstrated experience leading process change projects
- Presentation and seminar leadership skills

- Masters’ degree in health administration, nursing, or related field
- Registration with a recognized health professional college
- Lean Healthcare or similar certification
- IIBA or PMI Analyst certification
