**Position Description**:
The Community Support Services Supervisor is responsible for direct, staff organized and implemented programming and services that support older adult members of the community with specialized needs including Adult Day Services, Home Support Services and other client-centred services. This position directly supervises employees and volunteers to ensure services meet the communities’ needs while considering the core principles of inclusivity, accessibility and affordability. The Supervisor will create a welcoming and supportive environment that encourages socialization, and that also maximizes the independence of participants. As a member of the Community programming and Outreach team, the incumbent demonstrates support for the corporate values and participates fully in providing leadership and direction to the Section, Division and the Community Services Department.

This is a 15 month fixed-term contract opportunity,

**Accountabilities**:

- Daily supervision and evaluation of programs and services, including Adult Day Program, Home Support Services, and other specialized programs
- Develop, implement, and evaluate ongoing service specific programs and activities for a specialized aging population that require support and guidance in maintaining overall health and wellness
- Oversee program operations within legislation, regulations and approved policies and procedures. This includes compliance with accessibility and safety legislation (Occupational Health and Safety Act), Privacy legislation (i.e. PHIPA -Personal Health Information Protection Act, and other privacy acts (MFIPPA, FIPPA)), Connecting Care Act as well as all relevant safe work practices
- Recruitment, training, and supervision of staff; provide ongoing feedback and coaching; assists in annual performance evaluations for direct reports
- Development of work plans to achieve program goals and objectives set out in the department and master plans
- Administers program waitlist, intake and discharge functions. Completes initial client intake assessments for Adult Day Program and determines client suitability as per program guidelines in consultation with other staff
- Provides support, resources and community referrals for clients of the Adult Day Program and liaises with community agencies and other health care professionals to exchange client information as appropriate
- Updates and supervises the maintenance of client documentation/information, including proper authorization, documentation, storage and record keeping
- Coordinates services with community agencies (i.e. Home and Community Care Support Services) to ensure clients have an efficient, smooth transition within the health service system and to other community services as needed
- Supervise and support volunteer groups to implement acitivities and events for the Community Support Services area
- Ongoing assessment of community needs; research and analyze trends and best practices to develop programs accordingly
- Assist in the development of the section budget, monitor and report on budget variances on a regular basis, develop budgets for programs and special events, and monitor program expenditures to ensure budget compliance
- Knowledge of reporting formats, timelines, processes and statistical methods as per the practices of Ontario Health
- Liaise with staff from the Community Programming team and other departments as required
- Record and analyze program statistics for reporting purposes and to aid in program evaluation
- Develop and maintain a network with professionals in the field and staff in the community
- Liaise with education, business and other community-based stakeholders who are providing similar services
- Assist in the development of a marketing strategy for programs
- Provide support and back up to other staff in department as necessary
- Other duties as assigned

**Minimum Qualifications**:

- Post-Secondary diploma in a health related field such as Recreation, Social Work, Health Sciences, Gerontology, Occupational Therapy or related field
- Minimum 2 years’ experience working with and planning programs for seniors and adults in the community
- Knowledge of dementia and previous work experience supporting persons with dementia, mental health and addictions, including a strong understanding of the aging process and age or health related issues affecting older adults
- Previous supervisory/leadership experience in the community sector strongly preferred. Must possess strong leadership, motivation, delegation, coaching/mentoring and empowerment skills
- Knowledgable of community resources including various funding sources/grant opportunities
- Proven ability to establish and maintain productive working relationships internally and externally with a wide range of stakeholders and partnering organizations
