The Garry Hurvitz Centre for Community Mental Health at SickKids (GH-CCMH) is a child and youth mental health centre integrated with The Hospital for Sick Children (SickKids). GH-CCMH is undertaking an ambitious agenda to establish a revitalized academic community mental health centre that provides the highest quality of care, combined with excellence in teaching and community-based research. GH-CCMH at SickKids is located across two sites in Toronto and offers both in-person and virtual care to clients and their families, from prevention and early intervention through assessment, counselling and therapy and intensive services. We are also connected to the continuum of mental health services for children and youth supported by SickKids: the Brain and Mental Health Program, the SickKids TeleLink Mental Health Program, the SickKids CCMH Learning Institute and our online resources through AboutKidsHealth. **This position is initially dedicated to the transition of children, youth and their families from SickKids into GH-CCMH community based mental health services and supports. The position will work collaboratively with partners at SickKids to support the clinical transition pathway. The position may evolve to include other transitional pathways from acute to community care. This position will provide screening, triage, transition services, service planning and navigation to children and youth experiencing mental health concerns, and their families, to ensure they move in, through and out of publicly funded mental health services in a timely and seamless manner.**
**KEY RESPONSIBILITIES**
**Transition Services**
- Responds to referrals from SickKids for voluntary treatment services for eligible children, youth and their families.
- Obtains and reviews referral information and documentation determining eligibility of applicants.
- Conducts telephone or in-person intake interviews with parents/guardians and/or youth.
- Responds to requests for service information.
- Provides family education and support, as appropriate.
- Administers screening tools and, if needed, contacts SickKids for additional information.
- Develops effective working relationships with SickKids colleagues and collaborates/ communicates throughout the entire client transition process.
- Collaborates with Medical Director or designate regarding referrals.
- Triages and prioritizes requests for rapid response.
- Adheres to established service level agreements regarding referral acknowledgment and intake.
- Prepares client records entering information into a computerized client information system.
- Prepares information, statistics and reports for Information System Services and Program Evaluation Services, as required.
- Adheres to procedures for intake processing and the recording/distribution of client information.
- Assists with evaluating and modifying transition best practices and process improvements.
- Maintains up-to-date knowledge of industry trends, best practices and legislation
- Makes decisions related to intake eligibility and services, including outside agency referrals, within guidelines and policies, as well as refers unusual situations to the Manager.

**Working Conditions**:
- Works in a pleasant office environment
- Exposure to constant interruptions/distractions, deadlines, multiple demands and hearing difficult information about client experiences

**Qualifications**:
- Post-secondary Diploma (Early Childhood, Child and Youth Work or Social Services) and a minimum of one years’ related experience or equivalent combination of education and experience
- Membership in a professional college
- First aid, CPR and Behaviour Management Techniques knowledge and certification
- Good verbal and written communication skills to effectively deal with clients
- Good problem-solving skills to obtain and provide information to potential clients
- Demonstrated skills in therapeutic engagement and behavioural-based interventions
- Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of work
- Good knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint), as well as office equipment (telephone, scanner, photocopier and fax)
