**Summary**:
The Disability Resource Navigator will keep the family engaged and up to date with the available resources and supportive services such as Calgary Transit Access, Home Care, and specialized organizations for people with disabilities and continuously search for new resources. Collaborate with other service providers and families to build a solution-focused, family-driven plan for clients who face health-related barriers.

**Description**:
**Primary duties and responsibilities include**:

- Connect clients with disabilities and their families to appropriate resources available in the community
- Research supports available to clients with disabilities, both children and adults and their families familiarize with eligibility requirements and procedures
- Liaise with clients, Case Managers, families, care providers and community resources and provide follow-ups
- Observe, report, and assess clients’ progress; provide updates to Case Managers and the Health and Wellness team
- Maintain organized record keeping (while maintaining strict client confidentiality) to track services provided to each family with details of providers contacted
- May require occasional work on evenings and weekends and Some evenings as needed
- Assist families in identifying training opportunities to increase knowledge and skills to support their family member with special needs or themselves
- Conduct and facilitate presentations and training to build families’ skills and knowledge to support their family members who have a disability
- Actively participates in community resource planning and related work to establish better relationships with service delivery systems
- Collaboration with other stakeholders to help identify barriers for clients and their caregivers

**Qualifications**:

- Post-secondary degree/diploma in Education, Social Work, Human Services, or Health related field.
- 2-4 years of experience working with vulnerable populations, strong preference for experience with newcomers
- Demonstrated research and advocacy skills
- Excellent communication skills, ability to communicate in a second language considered an asset
- Proven ability to maintain strict confidentiality
- Computer literacy and proficiency with Microsoft Office programs essential
- Knowledge of the settlement and integration process and previous experience working with immigrant and refugee populations are considered a vital asset
- Knowledge of and willingness to learn about services, benefits, and supports available to families of children and adults with disabilities
- Ability to multitask and work in a fast-paced, multidisciplinary, culturally diverse environment
- Ability to coordinate and collaborate with families and service providers
- Able to perform duties independently and with mínimal supervision
- Must be flexible, reliable, proactive, collaborative and take initiative
- Must be legally eligible to work in Canada
- Must have proof of criminal record check with Vulnerable Sector Search
