Kinesiologist - Patient Navigator

**Schedule**: Monday - Friday, 2 evenings a week required, weekend hours may be required

**Location**: Primary Care Network Wellness Centre at #4000, 1800 194 Avenue SE; Calgary, AB
**Position Summary**
The Kinesiologist works in collaboration with patients, physicians and the other healthcare providers and is responsible for the evaluation, development, implementation, maintenance of safe and reasonable individualized and group exercise programs, and assisting patients with navigating through SCPCN’s Pain Program. They enhance the quality of life of their patients by promoting healthy lifestyles, teaching self-management skills to control health conditions and improving patient functionality. The kinesiologist will assist in facilitating seamless transitions between interprofessional team members and clinical settings for our patients.
**What Does the Role Do?**
Patient Navigation
- Pain Program referrals and ensures incoming documentation is complete
- Locates, collects, collates and analyzes pertinent patient information including previous diagnostic imaging or diagnoses, medication history, etc.
- Conducts initial patient intake appointment and Health History Reviews independently without physician support
- Assists patients in navigating through the inter-professional program with the goal of program completion
- Conducts follow up appointments and adjusts patient treatment pathways in collaboration with the interdisciplinary team
- Assists in evaluating effectiveness of patient care through individual patient assessment
- Reviews and evaluates patient evaluation surveys, records results in the EMR and advises interdisciplinary team of any changes to help determine treatment pathways
- Co-facilitates Pain Program specific workshops
- Actively leads and participates in program meetings including Clinical Rounds and Panel Review
- Collaborates and coordinates patient care with external stakeholders including other Pain services, referring physicians and Primary Care Behavioural Health Providers
Patient Care, Assessment and Treatment
- Reviews patient history and conducts physical fitness assessments for patient referred to our chronic pain clinic and GLA:D workshops
- Develops and implements an evidence-based physical exercise and fitness plans to meet patient fitness needs, abilities and goals, including evaluation of plans for effectiveness and safety
- Focuses on goal achievement, self-management and functional improvement
- Monitors patient compliance and outcomes
- Refers patients to other healthcare providers/agencies as required and provides relevant documentation
- Follows up with patients as required
Patient Education and Coaching- Promotes self-management of health conditions through motivational interviewing, coaching, education, skill development and resource navigation- Supports patients in carrying out their exercise plan by providing information, education, and training to increase patient knowledge
Group Facilitation
- Develops and implements group exercise activities and classes for patients with chronic pain
- Modifies exercises as required to meet individual patient needs
- Facilitates in-person and/or online patient group workshops including GLA:D as required
General duties- Documents assessments, physical exercise care plan and interventions in the EMR-
- Maintains and orders exercise equipment-
- Provides in-service training to other team members as needed-
- Communicates essential information to and collaborates with other primary care providers to support seamless case management and ensure smooth transitions along the continuum of care-
- Collaborates with the Program Manager and interprofessional team to identify and develop unique approaches to address patient needs and maximize patient care outcomes-
- Collaborates with Medical Home and other health care practitioners to ensure seamless care for patients-
- Mentors and orientates new staff to program/service area-
- Participates in meetings, ongoing program development, evaluation and quality improvement opportunities-
- Assists with implementation of new protocols, programs and acts as a resource for other staff-
- Supervises practicum students-
- Performs additional duties as required**Who Should Apply?**
- B.Sc. in Kinesiology with specific training in biomechanics, exercise, health physiology and leisure activity counselling
- Certified Exercise Physiologist (CEP) with the Canadian Society for Exercise Physiology or Exercise Specialist with the American College of Sports Medicine (ACSM)
- Current CPR certification and Standard First Aid certification
- Current professional liability insurance
- GLA:D Canada certification or ability to obtain same preferred
- At least three years' experience as a Kinesiologist, ideally in a primary care or ambulatory care setting dealing with chronic pain patients
