**The Role: Service Navigator (full-time)**
***The Service Navigator supports in the Triage and screening of referrals for the appropriate Trellis program, while maintaining program waitlists.
**Key Duties and Responsibilities**
- Triage and screen appropriate program referrals, and refer to alternative internal and external resources when not a fit for the program.
- Return phone calls with information about the program, explaining program service delivery and how to access.
- Manage and maintain the triage list in a timely fashion and assign families to the appropriate stream of services.
- Consult with program staff to gather and share pertinent information related to needs of families and capacity of program.
- Contribute to ongoing adjustments to triage & screening in response to environmental and systemic trends.
- Participate in and complete necessary training and mandatory agency related functions Participate in and contribute to a safe and healthy professional work environment.
- Effectively record and communicate relevant information at all times both internally and externally.
- Complete necessary paperwork expectations in accordance with protocols and timeline.
- Participate in ensuring outcomes are completed accurately and in accordance to reporting requirements.
- Be a representative of the agency when at community and collaborative events with stakeholders and other service providers
**Knowledge and Skills Expectations**
- Completed diploma or degree with at least 6 months of related experience.
- Experience working in human services field an asset.
- Knowledge of Calgary’s Social Services Sector and the resources available within it.
- Intermediate proficiency with Microsoft Office (Word, Excel, Outlook, Teams, PowerPoint) is required.
- Prior experience with calendar management and meeting logistics is preferred.
- Strong organizational, time management and planning skills to complete high volume of work in a fast-paced environment.
- Effective problem-solving skills, the ability to build collaborative working relationships, and the ability to complete tasks with high quality output.
- Experience working with individuals, families, and teams from a strength-based and trauma-informed approach.
- Ability to adapt through change
**Reporting Relationship**:
This position directly reports to a Trellis Program Manager.
