**POSITON SUMMARY**

The Health Promoter, as part of the interdisciplinary team, assists in the primary care functions, provides research and analysis, and has a key role in the development, implementation, monitoring and evaluation of programs and services of the West Nipissing Family Health Team (West Nipissing FHT) for individuals, families, and the West Nipissing community/Nipissing First Nation community.

The Health Promoter will work collaboratively with members of the team assisting in facilitation of improved preventive primary care and offering services to promote and preserve the health of clients and community. The role is responsible for identifying the health needs, planning, coordination, and delivery of services to specific populations, their families, and caregivers. The Health Promoter is actively involved in program development, implementation and evaluation for wellness and lifestyle programs and rehabilitation for patients living with chronic disease. They will also work with various community partners and undertake systematic reviews to determine the range of programs in existence and develop opportunities that exist. The Health Promoter will share expertise and knowledge of health promotion and disease prevention within the community helping to connect patients with appropriate services and professionals.

The Health Promoter conducts individual, group and community services based on needs assessments and follows up by coordinating, implementing, monitoring, and evaluating individual, and group sessions.

**DUTIES AND RESPONSIBILITIES**
- Develops, coordinates, implements, monitors, and evaluates comprehensive education/promotion strategies designed to meet the needs of the West Nipissing FHT clients and the community in accordance with the principles of health promotion and related competencies
- Interpret client health records and observe and record outcomes.
- Conducts individual and group needs assessments for health promotion and disease prevention (preventative care).
- In collaboration with Physicians, Nurse Practitioners and other IHP’s implement and discuss appropriate individualized or group care plan with the client/family care giver based on best practices.
- Facilitate linkages with appropriate services, supports, and resources.
- Supports the development, implementation, and evaluation of health promotion resource materials.
- Identify, manage, and develop health promotion program and disease prevention resources for all West Nipissing FHT members.
- Promotes awareness of West Nipissing FHT services and programs. Update website.
- Actively participates in staff, team, and committee meetings as appropriate.
- Provides leadership and mentorship to students that enter West Nipissing FHT on a placement.
- Participates in the development planning and evaluation of treatment, education, counseling, and health promotion activities of the West Nipissing FHT.
- Develops, organizes, and facilitates physical activity programs.
- Stays current and aware of opportunities to implement new, evidenced-based methods of client assessment, treatment, and programming.
- Participates in clinical projects / studies as required.
- Communicates effectively with health care team members to create a cohesive team and seamless services to the community.
- Communicates effectively with all clients, families, peers, other health care professionals and community partners.
- Enables individual and groups to participate in the resolution of their own issues.
- Contributing to the efficient functioning of the organization and the attainment of goals.
- Exercises reasonable care with caution in protecting confidential and sensitive information related to clients.

**JOB SPECIFICATIONS AND REQUIREMENTS**

**Education/Professional Requirements**
- Undergraduate or Masters Degree in health promotion field from a recognized University
- Certified Exercise Physiologist (CEP) or Registered Kinesiologist.

- Experience in leading group exercise classes, preferred
- Experience in program development, implementation, monitoring and evaluation
- Experience in health promoter role or related experience
- Require professional liability insurance
- CPR-C/Standard First Aid
- Smoking Cessation Certification (an asset)
- A demonstrated experience counseling/facilitating/delivery one on one group education
