**JOB OVERVIEW**
Working under the Director of Primary Care, the Intensive Community Support Advocate (ICSA) for the Manitoba HIV Program’s “Program to Access treatment for HIV and Support (PATHS)” is responsible for providing intensive case management for people living with HIV (PLHIV) in Manitoba who are not linked, or precariously linked to HIV care. The ICSA will provide a broad spectrum of effective and culturally appropriate services through an interdisciplinary outreach-based service model of care. Working within a trauma-informed and healing informed lens, the ICSA will promote empowerment, stability, recovery, and community engagement for individuals who have health concerns and/or are experiencing health issues. Drawing from AHWC’s mission statement and values, the person in the role of the ICSA will have the ability and skills to work with Constituents who may be oppositional and/or volatile, while continually working to maintain composure and behave in a professional and dignified manner.
The ICSA should demonstrate an understanding of and respect for the histories, traditions, and cultures of Indigenous peoples, inclusive of First Nation, Métis, and Inuit Communities and Nations, including an understanding of the importance of decolonizing healthcare systems and spaces.
**RESPONSIBILITIES AND DUTIES**
- Welcomes and supports all Indigenous - _First Nation, Inuit, and Métis _- and non-Indigenous community members entering AHWC or attached to AHWC, including people who identify as part of the 2SLGBTQIA+ community and persons who live with disabilities
- Nurtures a judgement-free, shame-free, social justice attitude, one that is focused on harm-reduction, social determinants of health, population health, health equity, and the journey of healing that the individual has defined for themselves
- Emphasizes culturally sensitive, culturally relevant, person-centered care in a competent, safe, and understanding manner
- Gives community members hope and encouragement
- Facilitates and provides outreach support to people who are HIV positive, not linked, or precariously linked to primary care, which can include non-traditional settings such as community centres, drop-ins, shelters, encampments, community agencies, residences, correctional settings, withdrawal management centres, hotels, parks, streets, and other primary care clinics
- Formulates and delivers individual tailored service, treatment, healing strategies and/or support plans to the community member, and in collaboration with the community member, based upon the community member’s personal beliefs and desires, including the provision of ongoing input, modification, and monitoring of progress/plans as needed
- Maintains an awareness of agencies, programs, and more that provide community and culturally based health, wellness, and lifestyle supports to members of the Indigenous community, for the purpose of promoting and providing appropriate community member referrals to ensure an effective integration of programs
- Coordinate and monitor referrals to community services and advocate for community members’ participation
- Arranges, escorts, and accompanies community members to medical appointments and errands as they relate to psychosocial supports
- Observes community member’s health condition and reports any concerns to HIV PATHS team (nurse and/or physician) or delegate
- Inspires safe practices in accordance with: AHWC’s program standards, policy and procedural operations, and organizational values; the HIV PATHS program and accredited standards; Regional Health and Provincial protocols
- Assists in response to emergencies by being familiar with the fire plan, emergency codes and procedures, safety protocols, and location of emergency equipment, etc.
- Coordinates security and safety plans with Security
- Problem solves and provides supports where needed, in collaboration with the HIV Paths team, Hub, and AHWC Leadership
- Liaises with community partners, Health Authorities, Cadham Provincial Lab, and Dynacare Lab services
- Reports operational issues to Director of Primary Care, Senior Director of Clinical Operations, and others as advised
- Maintains accurate record of outreach activities, adhering to documentation standards
- Attends staff meetings and other meetings as required
