Under the supervision of the Jordan Principle Service Coordinator, the Intake Coordinator will be responsible to coordinate all Off Reserve requests for the Southeast region, provide assessment, follow up, and liaise with other Jordan Principle Off Reserve offices.

**Responsibilities**:

- Assist individuals, families and communities to identify and deal with any denials, delays, disruptions of public services ordinarily available to children and families with special needs or needs not being met at home or community.
- Review all referrals made to SERDC Jordan's Principle
- Conduct assessments through telephone or in-person to confirm eligibility and determine appropriate services based on needs of the child
- Documents a coordinated intake assessment that includes referral, assessment, treatment/admission, discharge (if applicable), transition and ongoing care protocols. Communicates the service plan to children and family and appropriate indigenous community organizations.
- Provide navigation services with a cultural safety and child centered lens, using trauma informed and culturally competent approach.
- Build strong relationships and partnerships with frontline healthcare, childcare workers and other health professionals
- Develop and update resource list
- Any other duties to be assigned by the supervisor.

**Qualifications**:

- Bachelor's degree in social work, nursing or related field.
- Knowledge of population health and indigenous perspective on health and wellness.
- Excellent communication skills, including the ability to communicate effectively with children and youth from diverse backgrounds.
- Working knowledge of First Nations health approaches, protocols and culture
- Strong organizational skills and attention to detail.
- Ability to work independently and as part of a team.
- Ability to maintain confidentiality and handle sensitive information appropriately.
- Valid driver's license and reliable transportation.
- Advanced MS Office experience including Excel, Word, PowerPoint, Publisher, Outlook
- Ability to speak Ojibwe, an asset
