**Edmonton Southside Primary Care Network - Improvement Facilitator**

**Position Overview**:
The Edmonton Southside Primary Care Network is currently looking for a temporary Improvement Facilitator. This will be a 12 month contract. The Improvement Facilitator supports clinic teams to plan, implement, and evaluate practice improvement projects that target improvements in population health. The Improvement Facilitator liaisons with PCN member family physicians, clinic staff, Primary Care Managers, PCN multidisciplinary team (MDT) members, and the Quality Improvement team including Electronic Medical Records (EMR) Consultant/Coordinators, Panel Management Assistants, and PCN Evaluation, to work toward the overall goal of ensuring a high quality of evidence based health care to the practice population.

The Improvement Facilitator is a valuable member of the Quality Improvement team. The role of the Quality Improvement team is to align the goals of the Patient’s Medical Home and provincial indicators to those that are important and valuable at the practice level, with the end result of supporting quality care to the practice population. To be successful, the Quality Improvement team requires a high level of collaboration. Relationship building and data collection will inform the strategic direction for each clinic individually.

The Improvement Facilitator will report to the Quality Improvement Manager.

**Key Responsibilities**
- Lead quality improvement discussions with physicians and clinic teams to develop processes that support Medical Home optimization including panel management, access improvement, team-based care, and delivery of planned care for groups of patients.
- Engage physician members and clinic teams to develop quality improvement plans that align with the objectives of the Patient’s Medical Home.
- Facilitate clinic teams to develop, implement, and evaluate improvement processes identified by the physicians and clinic team, as well as projects originating within the PCN.
- Be a leader and a champion for change management initiatives.
- Develop, organize, and facilitate learning opportunities for physicians, clinic teams, and PCN staff to support objectives of the Patient’s Medical Home.
- Act as a liaison between the PCN and other provincial initiatives that support quality improvement and Medical Home transformation.
- Provide support to Panel Management Assistants and assist with their integration into clinics, as well as role optimization and reporting.
- Work collaboratively with Primary Care Managers to identify quality improvement engagement strategies for each clinic.
- Maintain quality improvement partnerships, both with internal team members and external stakeholders. A key deliverable in this role will be supporting adoption of Community Information Integration and Central Patient Attachment Registry (CII/CPAR) platforms within Medical Homes (ESPCN member clinics).

**Competencies and Behaviours**
- Undergraduate degree in healthcare, health promotion, business, and/or the social sciences, or related field. A Graduate degree is preferred.
- Vehicle/license.
- Understand Alberta privacy laws and act according to those laws.

- Previous experience:

- Familiarity with the health system, including primary care and clinic cultures.
- 3 or more years’ experience in change management, group facilitation, and/or
project management
- Proficiency with Microsoft Office programs.
- Knowledge and experience with process improvement methodologies.
- Basic understanding of the purpose and use of multiple EMR systems.
- Interpreting data and using tools to communicate trends and inform organizational decisions.
