What You Will Do
- Provide direct outreach services to people living with, affected by HIV/AIDS to address issues of personal health and well-being, and to improve connection to treatment and access to appropriate community services through a culturally informed lens.
- Conduct intake and needs assessments to develop client centered approaches to care in community settings, including hospitals and clients' homes to maximize accessibility of services.
- Provide support to individuals residing at our John Gordon Home program with a primary focus on ensuring individuals are engaged in securing stable housing well before discharge.

What You Will Bring
- A post-secondary degree or diploma in social services, social sciences, or a related field and/or 2 years of work experience in case management, supportive counselling services or a related field.
- A strong knowledge and understanding of the impacts of stigma and oppression on people who are living with, at risk of or impacted by HIV/HCV, may be vulnerable and marginalized, and may be experiencing challenges related to anti-Black and anti-Indigenous racism, mental health, addictions, and homelessness.
- An enthusiasm for conducting community-based outreach and meeting folks where they are at, both emotionally and physically.
- A strong ability to build rapport with clients and engage with and relate to culturally, linguistically, ethnically, spiritually, racially, and geographically diverse communities.
- A strong knowledge of community resources and social service agencies in London with the ability to identify and understand intersectional factors that impact the health and wellbeing of diverse communities of people, including the impacts of anti-Black and anti-Indigenous racism.
- Health education and promotion knowledge and skills, including program development and implementation skills, group facilitation and presentation skills, and community event coordination/delivery and community engagement skills.
- Excellent interpersonal, communication and problem-solving skills with the ability to deliver effective conflict resolution and crisis intervention techniques.
- The ability to occasionally work irregular hours such as evenings and weekends in a variety of social and physical settings.
- A valid driver’s license with the regular use of a reliable vehicle for work-related duties is preferred.
- First Aid/CPR, BLS and Naloxone training are considered an asset.
- A satisfactory vulnerable sector police check (VSC).
