As these are primary care focused roles, both positions require a commitment to on-site work in our local communities, travel between Britannia Beach and Pemberton is required.

**RESPONSIBILITIES**:

- Provide strategic leadership in the implementation of PCN initiatives, collaborating closely with stakeholders to ensure alignment with organizational goals.
- Develop and implement operational procedures to facilitate the consistent delivery of services across the Sea to Sky PCN.
- Monitor and report on the progress of PCN initiatives, ensuring compliance with Ministry of Health deliverables and reporting requirements.
- Foster engagement with local physicians, nurse practitioners, and community partners to drive participation in PCN activities and initiatives.
- Lead project management activities, including proposal development, budgeting, implementation planning, and evaluation.
- Collaborate with stakeholders to identify and monitor performance indicators, preparing reports and presentations as required.
- Proactively identify opportunities for process improvement and innovation to enhance the quality and accessibility of primary care services.
- Participate in the recruitment, selection, and orientation of PCN staff, providing ongoing support and mentorship as needed.
- Cultivate a culture of collaboration and teamwork, promoting effective communication and conflict resolution strategies.
- Develop and implement communication strategies to raise awareness of PCN resources and services among healthcare providers and the broader community.
- Support change management initiatives to facilitate the successful implementation of PCN objectives and initiatives.

**QUALIFICATIONS**:

- Post-secondary degree in a health-related discipline, with a preference for a Master's degree in relevant fields.
- A minimum of five (5) years of progressive leadership experience in healthcare settings, with a demonstrated track record in project management and team leadership.
- In-depth understanding of primary health care principles, population health, and familiarity with Primary Care Networks and the Patient Medical Home model.
- Knowledge of Indigenous cultural safety and humility, coupled with an understanding of relevant governmental policies and initiatives.
- Exceptional communication, interpersonal, and organizational skills, with a proven ability to navigate complex stakeholder environments.
- Strong analytical and problem-solving abilities, with a keen eye for detail and a commitment to continuous improvement.

**Salary**: $110,000.00-$120,000.00 per hour

Schedule:

- Monday to Friday
