Radius Community Health & Healing provides comprehensive, person-centered primary health care for vulnerable Edmontonians with complex needs.
Through primary care and health promotion, Radius Community Health & Healing improves the quality of life and well-being of populations and individuals who face multiple barriers to accessing health services.
**_ Job Description_**
Health Advocates work within a multi-disciplinary team to provide social support to the vulnerable population in Edmonton. Clinical teams offer comprehensive, wrap-around care to Edmonton’s most marginalized population. Health Advocates work directly with clients to navigate social and healthcare systems.Radius clients are often faced with barriers of stigma, and the role of the Health Advocate may often be hands-on and will require direct advocacy and intervention on behalf of clients.
**_ Key Responsibilities_**
- Connection to essential services in the health and social sector
- Assist clients in navigating available social supports (including financial, housing, food security, and other basic needs)
- Collaborating with other healthcare providers to achieve client-centred goals
- Assist clients to secure basic needs, documents, and services (e.g. identification)
- Client advocacy
**_ Qualifications_**
- Graduate of an approved Social Work program
- Registration and good standing with the ACSW
- Experience working with marginalized populations.
- Clear criminal record and vulnerable sector check
- Current CPR certification
**_ Skills and Competencies_**
- Ability to work collaboratively within an interdisciplinary team.
- Flexibility in organizing and managing caseloads.
- Ability to adapt to unexpected challenges and conflicting priorities throughout the workday
- Excellent verbal and written communication skills
- Demonstrates initiative, good judgment, and resourcefulness.
- Strong problem-solving skills
- De-escalation skills are an asset.
- Non-judgmental and respectful of various individual, family and community lifestyles
- Knowledge of and ability to deliver trauma-informed care
- Knowledge of and ability to deliver care through a harm-reduction lens
- Cultural awareness
- Strong computer proficiency, with experience with Office 365 suite (e.g. Outlook, MS Office, Excel) an asset
- Prior experience with electronic medical record (EMR) systems an asset (e.g. MedAccess)
