**Case/Outreach Worker**

TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking a **Case/Outreach Worker **to join our team in a **permanent** **full-time **role.

**DUTIES & RESPONSIBILITIES**
- Develop and maintain the network of resource professionals for program support.
- Collaborate and coordinate with Trades Connect partners to develop in-house case management capacity.
- Intake clients and assess additional support required (Mental Health, Childcare, transportation etc.)
- Coordinates and monitors technical and on-the-job training.
- Conduct investigations on compliance issues and prepares reports.
- Promote the Apprenticeship and Trades Qualification program; and monitors quality of technical and practical training.
- Ensure and monitor that program activities and the staff's approach to our clients are adhered to our client bill of rights.
- Prepare case management related reports including but not limited to: outcomes, successes.
- Conducts crisis intervention as needed.
- Ensure that policies and procedures of the agency are implemented and adhered.
- Collaborates and coordinates with other programs and coordinators on behalf of the Orientation and Welcoming Communities Program.
- Administrative functions including administration of agreements, training and examinations
- Develop profiles for clients to help identify any wrap around supports
- Assist the staff with providing relevant information and support the staff for resources and other services
- Jointly with service partners, conducts program promotion in community social landmarks Supports outreach activities with other TNO departments and services
- Attend outreach events and represent programming

**Screening** - As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

**Job Type**:Full-Time permanent position

**Work Location(s)**: 45 Overlea Blvd.Toronto, ON M4H-1C3 and other TNO locations

**Schedule**: Monday to Friday, 35 hours a week - Some evenings and weekends may be required. Typically 9:00 AM to 5:00 PM or 11:00 AM to 7:00 PM, based on program needs.
