Social Worker

Counsel individuals to maximize the independence and employability of persons coping with personal, social and vocational difficulties that result from birth defects, illness, disease, accidents, or the stress of daily life. Work under the supervision of a psychologist as part of a multidisciplinary team to provide consultative social work services in accordance with HBHN protocol and established standards of professional practice.
**Responsibilities**
- Assess client needs, design and implement rehabilitation programs. Confer with physicians, psychologists, physiotherapists, occupational therapists, and other professionals to develop and implement client rehabilitation programs
- Collaborate with Health Professionals who work within a counseling and case management framework to assist people who are experiencing disability
- Arrange for physical, mental, academic, vocational and other evaluations to obtain information for assessing clients' needs and developing rehabilitation plans according to HBHN protocol
- Handle case assignments, draft service plans, and review case progress according to HBHN protocol
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help patients make informed decisions by acting as their advocate, regarding their clinical status and treatment options and assisting them in seeking referrals
- Develop effective working relations and cooperate with medical team throughout the entire case management process
- Arrange abeyances with social services, health and governmental agencies. Research appropriate services and programs
- Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
- Record case information, complete accurately all necessary forms and produce statistical reports
- Adhere to professional standards as outlined by protocols, rules, and regulations
- Ongoing Communication with Treatment Providers, Patients and Family Members
- Ongoing Review of medical, background documentation, and records
- Represent the company in events, conferences
**Requirements**:
- BS degree in rehabilitation healthcare field, preferable in Social Work
- Proven working experience in the healthcare rehabilitation field
- Knowledge of case management principles and healthcare management
- Previous experience with rehabilitation and psychological aspects of care
- Effective communication skills
- Excellent organisational and time management skills
- Familiarity with professional and technical emerging knowledge
- Problem-solving skills and ability to multi-task
- Compassionate with teamwork skills
- Current license, certification, or registration
Pay: From $30.00 per hour
Expected hours: 40 per week
