**PERSONAL PLAN FACILITATOR**

**South District, (London, Windsor, Chatham)**

The Personal Plan Facilitator works with the person using services, their circle of support (including the Christian Horizons’ team(s) that provides support) to learn about the person and use this information to facilitate the creation of a comprehensive Personal Plan. The plan will address the person’s needs, hopes and dreams with an eye toward building communities of belonging in accordance with the core Values of Christian Horizons; Valuing People, Fostering Belonging, Serving Others and Respecting Gifts. The Personal Plan Facilitator will respect and uphold the Values, Vision and Mission of Christian Horizons, demonstrating the highest standards of courtesy, respect, and integrity in their dealings with people using services, other employees and both internal and external stakeholders.

**Requirements**:

- 5 years experience in Developmental Services or other similar social services
- University degree
- Strong communication skills (written and oral) and interpersonal skills
- Strong facilitation skills
- Satisfactory Vulnerable Sector Police Check
- Confirmation of COVID vaccination
- Full G Drivers Licence and access to reliable transportation

**Job Types**: Part-time, Fixed term contract

**Salary**: $22.92-$26.87 per hour
