Position: Program Manager, Permanent Full-Time

Participation House Toronto is a not-for-profit charitable organization providing supportive housing to developmentally disabled adults in the greater Scarborough area. In providing services to persons with developmental disabilities, our goal is to provide an opportunity for each client to live in a safe and supportive environment and to participate to their fullest potential in all aspects of community living.

Participation House Toronto is hiring a Program Manager to provide support and engagement to our developmentally disabled adult clients, their families, and our staff in residential settings in the Scarborough area. Primary duties and responsibilities include:

- Create and implement Individual Service Plans for all clients on an annual basis, including quarterly reporting, annual goals, care planning, behavioural plans, and other applicable documentation
- Implement and monitor the legislative and compliance objectives of the Ministry of Children, Community, and Social Services and Ministry of Health and Long-Term Care
- Ensure client medical needs are being met, including initiating and managing external client supports and attending medical appointments
- Co-ordinate and supervise DSW and PSW student placements
- Provide assistance and guidance to families with issues regarding client care, programming, annual goals, and other service delivery issues
- Facilitate recreational programming ranging from one-to-one, small group, large group, and community based outings while ensuring program participation and engagement
- Perform personal care duties as required, which may include but not be limited to, client feeding, client supervision, assistance with mobility, and assistance with medications.
- Prepare monthly programming calendars and newsletters based on individual ISP goals and group preferences for distribution to clients and families
- Communicate and collaborate with external agencies providing services to clients, including day programs, medical specialists, and other service providers
- Manage community transportation needs for clients
- Manage conflict resolution and problem solving regarding individual client issues
- Communicate with and support front-line staff and students in relation to client needs, programming, and scheduling
- Schedule and supervise annual and ongoing training for front line staff
- Creation of seasonal menus to suit client dietary needs and tastes in accordance with the Canada Food Guide
- Monitor the appearance, condition, cleanliness, and organization of all sites in conjunction with the Manager of Client Services
- Conduct monthly audits of client medication, medical supplies, and care supplies and ensure new inventory is ordered in a timely fashion
- Track and audit client Personal Needs accounts and provide documentation to the Manager of Client Services in compliance with the QAM standards
- Track and audit client program expenditures from individual annual program budgets and Passport support funding
- Other duties as required for the effective and efficient functioning of the agency

Qualifications for the position include but are not limited to:

- Graduate of an accredited post-secondary program in an applicable field, including Gerontology, Therapeutic Recreation, Health Sciences, or equivalent experience
- Demonstrated knowledge and experience in the Developmental Services, Community Support Services, or Long-Term Care sector
- Demonstrated knowledge and experience working with developmentally disabled adults or seniors
- Demonstrated management and organizational skills, creativity, and high motivation
- Demonstrated written and verbal communication skills
- Ability to work flexible hours as required including some evenings and weekends
- Ability to manage conflict resolution and group dynamics
- Infection Prevention and Control training
- Crisis intervention and de-escalation training (CPI or Safe Management preferred)
- Valid CPR and First Aid certification
- Completion of a successful Vulnerable Sector Screening Check
- Valid driver’s licence, appropriate liability insurance, and access to a reliable vehicle
- Proof of full COVID-19 vaccination status
- Must be able to independently reposition/push/pull clients (approximately 40-90kg) using safe body mechanics
- Job may require continuous walking, use of stairs, stretching, bending, and pushing of wheelchairs
- Experience working in a unionized environment an asset
- Fluency in French is an asset

This is a permanent full-time position, reporting to the Executive Director as part of the management team of Participation House Toronto. Salary is commensurate on qualifications and experience.
