**Who We Are**

With a demonstrated commitment to the values of Responsibility, Availability, Adaptability, Hospitality, Quality and Dignity, the Intensive Case Manager, (ICM) will be responsible for the provision of direct service to clients who experience mental illness, engage in substance use and who may experience barriers to resources for health, wellbeing, vocation and education. The professional relationship between the client and ICM, is grounded in the regular assessment of client centered needs and is a collaborative effort of the client and the ICM in the pursuit of client driven goals.

The **Intensive Case Manager** will demonstrate a wide array of strengths including, a compassionate and caring nature, in conjunction with an unbiased, non-judgmental and person-centered approach while working with clients to develop a level of trust in order for clients to feel secure and supported. This level of trust will be established and maintained by involving the client as much as possible in discussion around their goals and recovery process.

Through the provision of a positive work ethic and effective role modelling, the ICM is responsible for the demonstration of the Mission, Vision and Values at work within Good Shepherd Non-Profit Homes Inc. Toronto.

**How You Will Make an Impact**
- To carry out all duties with a work ethic demonstrating leadership, role modeling of behaviours and managing responsibilities in accordance with the Mission and Values of Good Shepherd Non-Profit Homes.
- To provide service in a dignified, culturally-competent and respectful manner that reflects the charism of Good Shepherd Centres and which reflects an integrated anti-racism/anti-oppression framework.
- Maintains a case load within the provincial guidelines for Intensive Case Management.
- Engages in the intake process including, liaison with Access Point, interviewing potential clients, utilizing appropriate assessment tools to determine the goals and needs of clients, client summary reports, client discharge/transfer reports, maintaining client files, client progress notes and completion of all other necessary paperwork and assignments.
- Provides resource information, community facilitation, and advocacy for clients as needed. Completes referrals as required.
- Utilizing a psychosocial rehabilitation approach to care, supports clients in achieving goals that enhances the client's enjoyment and quality of life.
- With client consent, liaises with clients' formal and informal supports.
- Assists to enhance and develop necessary life skills as needed.
- Assists the client to maximize their competencies, in order to become more self-sufficient
- Works from a strengths-based perspective within an empowerment framework.
- Keeps the Program Manager informed of developments and changes concerning clients on an as needed basis.

**What You'll Bring to Our Team**
- Post-secondary education, preferably related to Social Services and the Mental Health and/or Addictions.
- 1-3 years experience in community mental health/addictions preferred.
- Knowledge and understanding of service delivery models i.e harm reduction strategies, trauma informed care.
- Experience with crisis resolution, crisis prevention intervention.
- Strong knowledge of community resources relevant to service.
- Excellent Communication (both verbal & non-verbal), interpersonal and organizational skills.
- Ability to work independently as well as a team player.
- Ability to work with an inter-disciplinary team member's.
- Ability to work well under pressure.
- Ability to take direction from clients and advocate on their behalf.
- ability to demonstrate flexibility and a demonstrated high degree of interest in the field. of health care with particular emphasis on community-based programs.
- Experience working with psychiatric patients and housing programs preferred.
- Knowledge of vocational, psychosocial, medical, educational community resources, crisis care facilities and housing resources in Toronto.
