**Job Summary**:
This role is an integral to coordinating program development and administration of projects, including the Long Term Care Initiative (LTCI). This role influences the transformation of long-term care in Richmond, in collaboration with Richmond family physicians and our Vancouver Coastal Health - Richmond (VCH - Richmond) partners. The Program Coordinator reports to the Program Lead. This role also supports family practices, also referred to ask Patient Medical Homes (PMH’s) with their general practice support needs.

**Organizational Status**

The Richmond Division of Family Practice (RDFP) is a local non-profit society that is part of a province wide initiative designed to improve patient care, increase family physician influence on health care delivery and design, and enhance professional satisfaction for physicians. The Richmond Division has over 220 physician members and is led by a Board of Directors, and works in collaboration with Vancouver Coastal Health, BC’s Ministry of Health, and community organizations.

In 2017, the Long-Term Initiative (LTCI) project was launched in Richmond to improve patient care, increase family physician influence on health care delivery and design, and enhance professional satisfaction for physicians. The Richmond Division of Family Practice coordinates Richmond long-term care initiatives across 5 care homes: Fraserview, Lions Manor, Minoru, Pinegrove and Rosewood.

**Key Accountabilities and Responsibilities**

Works closely and efficiently with LTCI family physicians, LTC home physician leads & program lead, VCH-Richmond, GPSC and other regional partners to coordinate Richmond LTCI program development, administration, and quality improvement projects

Collaborates with Vancouver Coastal Health-Richmond and other stakeholders to align efforts to support physicians working in long-term care

Attends interdivisional LTCI meetings twice a year and work with other LTCI Program Coordinators in the lower mainland

Prepares minutes, agendas, logistics, and other documentation and facilitates regular meetings:

- Steering Committee
- QI Working Group
- Facility visits

Works closely with the Division LTCI Physician Lead to support program goals in alignment with best practices of care and system level outcomes defined by the program

Prepares monthly reports and shares with stakeholders

Works with the Richmond Division Communications Coordinator to create engagement materials (website segment, profiles, handouts, media, etc.) about the LTCI program.

Manages invoicing and prepares quarterly rostering payments

Prepares a 6-month call group schedule and tracks weekly on-call stats

Collects and analyzes GPSC QI facility survey responses & program data from facilities

Plans the LTCI CME (learning event) schedule and make all arrangements

Recruits new family physicians into LTCI in collaboration with VCH-Richmond and LTCI Medical Leads and Administrative Leads

Updates LTCI family physician profiles for LTC home teams upon new physician orientation to the LTCI program

Co
- facilitates members’ orientation meetings and mentorship with LTC home Physician Leads and Administrators

Handles sensitive information in a confidential manner

Other duties as assigned

**Primary Medical home (PMH) Work Performed**

Engages with Richmond Division of Family Practice family physician members, providing new members with orientations, and navigating family physician members with practice support inquiries

Supports with maintenance of member and clinic records within an RDFP database

Collaborates with the RDFP Communications Coordinator to maintain membership resources on member facing media platforms (website, member portal, etc.)

Maintains membership activity associated workflows

Collects, analyzes, and reports on membership activity data

Coordinates physician networks and peer mentorship

Supports with membership special projects

Other duties as assigned

**Working Conditions**

The Program Coordinator position requires availability to work from Monday to Friday during regular working hours with the possibility of attending occasional early morning, early evening and/or weekend meetings. Frequent travel may be necessary across Richmond/Vancouver area requiring reliable transportation.

**QUALIFICATIONS**
- 2-3 years minimum administrative experience
- Previous experience working within the BC Health Care system
- Minimum Bachelor’s degree in Health Sciences, Business Administration, or related topic area
- Must have advanced knowledge of MS Office (specifically Excel database management)
- Excellent written and verbal communication skills, with attention to details
- Highly developed organizational and time-management skills, with the ability to meet weekly and monthly deadlines
