Position Summary
Clinical Coordinator, Orthopaedic & Plastic Clinic, Ambulatory Care
The Clinical Coordinator role is a leadership position within the Ambulatory Care Services with the primary focus on the collaborative management and enhancing the patient experience and flow. Embracing models of operational and clinical excellence, patient and family driven care, you will in partnership with the interdisciplinary team, other teams across the hospital and external partners (including the LHIN, surgeons’ offices, and other hospital groups). You will provide leadership in rapidly changing patient situations and is an autonomous professional who works independently and interdependently supporting the health care team. You will also act as a resource to patients, families and members of the care team by providing consultation in situations where advanced nursing expertise is required; provide advice and guidance to other health care providers regarding complex patient care matters, planning, policy development and problem solving. You are highly organized, able to succeed in a fast paced and high volume client facing environment, and are eager to engage with new challenges every day. You also have well-developed interpersonal, critical thinking and problem solving skills, are able to work independently as well as within a multidisciplinary team, are able to effectively prioritize and multi-task. You have excellent verbal and written communication, teaching skills and project management skills. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families. This position supports the Hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. On a practical level, you will
Work collaboratively with CTM, Educator, in terms of daily workflow and staff schedule
To be a liaison for patients and families. Facilitates patient flow and utilization of appropriate resources
Supports the learning needs of all disciplines and collaborates with other program clinicians to implement change to meet identified needs
Responsible for project management and coordination of unit and corporate based initiatives
Manage clinical responsibilities, such as day to day assistance for staff nurses, inter-professional team
Possess understanding of procurement process in a clinic setting to support the process
Develop systems and processes, in partnership with the team, for effective unit functioning including patient booking process
Assist in the implementation and evaluation of targets, goals and objectives as identified both internally and externally
Support staff in utilizing critical thinking skills in resolving clinical and administrative issues
Provide direction and guidance to all members of the health care team, patients and their families, groups and organizations and collaborate with the health care team members to plan for the care of patients and their families
Engage in evidence-based practice and may initiate changes to improve the quality of patient care as appropriate (including the planning, implementation and evaluation of the introduction of ‘best practice’
**Qualifications**:
Possess a valid Certificate of Competence from the College of Nurses of Ontario
BCLS certified, ACLS an asset
Baccalaureate in Nursing, Health studies or administration, masters in nursing, preferred
3-5 years of experience in a Surgical and/or Ambulatory setting
3-5 years leadership experience (Charge Nurse) preferred
Teaching experience, preferred
Well-developed knowledge of Ambulatory or Surgical Services, clinical nursing practice, research methodology, data collection and analysis
