This is a full time contract position. With On call rotation, Working in our Clinton or Stratford office. This position includes supervising, providing direction, education, and guidance to Personal Support Workers who provide personal support services to clients in the community. The primary focus of this position is to provide mentoring to our PSW students. Developing goal-oriented, outcome-based care plans for clients is an integral component of the work as well as coordinating, planning and monitoring services. You will act as a liaison with all referral sources from our community partners.
**Qualifications**:
- RPN with current certificate of competence from the College of Nurses of Ontario
- 2 years supervisory experience, with ability to coach, develop employees, facilitate team work and resolve conflict.
- Previous PSW Experience
- Demonstrated willingness and ability to be adaptable, build relationships, focus on consumer needs, communicate effectively
- Exceptional assessment and problem solving skills
- Excellent interpersonal, communication, time management, computer skills
- Valid Ontario drivers licence with ability to travel in Huron or Perth County
- French Speaking would be considered an asset
