**What You Will Do**

In this role, you will get to work as a leader within the Client Services department of Regional HIV/AIDS Connection, specifically out of John Gordon Home (JGH). Within this capacity, you will manage front-line staff and coordinate the care needs of residents living within JGH.

In this work, you will:

- Develop, implement and monitor, evaluate and report on the individual resident care plans, liaising with community agencies, health and social service professionals.
- Complete assessments, reports and other funder requirements for residents, as required.
- Coach and support residents to achieve their goals; provide case management support.
- Coordinate transition plans for residents moving into and out of JGH, in collaboration with RHACs Case Managers, on site JGH staff and community partners.
- In consultation with department Director, interview, select and exit residents of JGH
- In consultation with department Director, recruit, select, orient, train, motivate and manage the performance of various positions within JGH

**What You Will Bring**
- Minimum 5 years management experience, preferably in non-profit agency
- Post-secondary degree or diploma in social services, social work, nursing and/or related field
- Case management and crisis intervention knowledge and skills
- Knowledge and skills related to working with people who are living with, at risk of or impacted by HIV/HCV; who may be vulnerable and marginalized; and may also be experiencing challenges related to mental health, substance use and homelessness
- Social services knowledge and skills, including availability of community resources
- Program design, delivery and evaluation knowledge and skills
- Knowledge of relevant employment legislation, including occupational health and safety
- First Aid/CPR, BLS and Naloxone training considered an asset
- Satisfactory vulnerable sector police check (VSC)
