Home Health Service Specialist

The Home Health Service Specialist role plays a crucial role in ensuring that individuals who require home care services receive the assistance and support they need to maintain their health and well-being while living in their own homes. This position provides one-to-one services with clients, whether it be located at Harbour Landing Village, or in their homes. As the client base continue to grow, this position will take a leadership role in supervising a growing team of companions, homemakers, and personal care workers. It's important for individuals in this role to have strong communication skills, patience, empathy, and a commitment to improving the quality of life for those they serve.

**3.** **Responsibilities**:
As the Home Health Service Specialist, your responsibilities will include, but are not limited to:
**A.** **Services Provided**:
- Provide direct client care as needed in a variety of roles such as:
a. **Personal Care**: Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, medication reminders, toileting, and mobility.
b. **Homemaker Assistance**: Help clients with household tasks such as meal preparation, light cleaning, laundry, and grocery shopping. Ensure a safe and clean-living environment.
c. **Emotional Support**: Offer companionship and emotional support to clients. Engage in conversation, provide a friendly presence, and help alleviate feelings of loneliness or isolation.
d. **Transportation**: Provide transportation for clients to medical appointments, social outings, or other necessary errands.
e. **Day Program**:Support and provide assistance with our on-site Day Program, which offers a range of group activities and support services tailored to the needs of our senior participants.

**B.** **Supervision and Mentorship**
- As the business grows, this role will lead and supervise a team of companions, homemakers, and personal care workers, providing guidance, training, and support.
- Collaborate with management to participate in the recruitment, selection, and training of new companions, homemakers, and personal care workers.
- Help onboard new team members, ensuring they receive proper training and orientation.
- Conduct regular team meetings to discuss client care plans, best practices, and address any concerns or questions.
- Conduct supervisory shifts with employees for on-the-job training and periodic performance appraisals.
- Ensure continuity of care for clients during staffing transitions and while the team is growing.
- Complete first visit with a new client to understand first-hand what is expected and fill in shifts as needed.
- Implement safety measures and precautions to prevent accidents or injuries in the client's home. This may include fall prevention, infection control, and emergency response protocols.
- Ensure compliance with all relevant laws and regulations governing home care services, including HIPAA (Health Insurance Portability and Accountability Act) for patient confidentiality.
- Demonstrate a caring and empathetic attitude towards clients, understanding the challenges they may face due to illness or disability.
**C.** **Documentation**
- Collaborate in the creation and maintenance of detailed documentation of roles, responsibilities, and expectations for companions, homemakers, and personal care workers.
- Document client-specific care plans, scenarios, and any modifications made during the care process.
- Maintain accurate records of client progress, staff performance, and incident reports.
- Utilizing the online database, ensuring client information is accurate and being used properly.
- Provide training on platform to caregivers, answering any questions about charting or documentation.
