Reporting to the Vice President Medicine, District Stroke Centre, HIV Clinic, IPAC, Pharmacy, Access and Flow the Manager of the District Stroke Centre Manager coordinates stroke care priorities across the continuum of care in Windsor-Essex. In alignment with the regional stroke network work plan contributes to the development and implementation of a cross-continuum district work plan in collaboration with key system partners.

**RESPONSIBILITIES**:

- Support the District Stroke Centre to achieve its Service Guideline accountabilities to develop, implement, coordinate and integrate stroke care throughout the stroke district and across the care continuum.
- Develop and foster effective working relationships with internal and external stroke system partners to enable the advancement of stroke district priorities.
- Lead and support implementation of best practice, in collaboration with health system providers within the district, across the continuum of care, including actively managing and supporting change initiatives/projects.
- Represent the interests and needs of the stroke district at local, regional, and provincial tables (e.g., Regional Stroke Network Steering Committee, Regional and District Advisory Committee, Ontario Health Team committees/groups).
- Actively share information relevant to the stroke district/local context from forums/committees with internal and external stroke district partners/team to promote knowledge transfer and exchange.
- Identify district specific needs for provider education and facilitate the regional education activities in conjunction with the Regional Education Coordinator.
- The DSC Coordinator will provide leadership and support in the Secondary Stroke Prevention Clinic for follow-up care and preventive interventions for high risk stroke patients ensuring close linkages with primary care, acute care, and stroke rehabilitation programs.
- Support measurement and monitoring processes by working with partners to define data needs, collect data, assess performance through monitoring data trends, evaluate outcomes and develop standards throughout the district and across the continuum of stroke care.
- Administer and manage associated funds and resources to support the stroke district work plan and system improvement initiatives, as appropriate.
- Other duties as assigned

**QUALIFICATIONS/SKILLS**:

- Minimum undergraduate degree from a health science related discipline; Maters Degree in any field preferred
- Three years minimum clinical experience and recent leadership experience in healthcare
- Demonstrated facilitation, change management, conflict resolution, project management and coordination skills
- Thinks strategically; creates strategic goals; defines, plans, and promotes a clear vision for the future.
- Demonstrated ability to support and embrace the philosophy of Service Excellence and Patient and Family Centered Care.
- Provision of care in an inter-professional team environment.
- Excellent interpersonal skills for collaboration, coaching, counseling and conflict resolution.
- Proven flexibility with a high level of initiative and self-direction.
- Experience in both hospital and community service delivery is an asset.
- Experience in related clinical practice.
- Excellent computer skill and Strong working knowledge of Microsoft Office computer programs.
- French Language proficiency an asset.
