Position Summary

**Brain Injury Service Coordinator**;
Reporting to the Program Coordinator, the Brain Injury Service Coordinator is responsible for the coordination of supports and services to Individuals with Acquired Brain Injury in accordance with Blue Heron’s philosophy, the Principles of Support and the Individual’s Personal Support plan. The Brain Injury Service Coordinator is responsible for providing information and resources to the community, promoting awareness and ensuring service delivery/availability for Individual’s requiring support.

The Brain Injury Service Coordinator works closely and in consultation with the Program Coordinator, the Individual and their support team and the community.

The Service Coordinator provides support and supervision to Supports for Community Living (SCL) staff working in the program on a consistent and regular basis to ensure the quality of services being provided. The Service Coordinator monitors the supports provided by contracted service agencies to ensure services are delivered based on the individual’s service plan and in accordance with the service agreement with the contracted agency.

Position Summary

Reporting to the Program Coordinator, the Community Access for People in Continuing Care(CAPCC)Service Coordinator is responsible for the coordination of supports and services to Individuals under the age of 65 and living in a long-term care facility in accordance with Blue Heron’s philosophy, the Principles of Support and the Individual’s Personal Support plan. The CAPCC Service Coordinator is responsible for working with the Continuing Care facility and staff to coordinate community access services considering individual’s health needs. The CAPCC Coordinator provides information and resources to the Continuing Care staff, families and community, promoting awareness and ensuring service delivery/availability for Individual’s requiring support.

**CAPCC Service Coordinator**

The CAPCC Service Coordinator works closely and in consultation with the Program Coordinator, the Individual, their support team and the community.

The CAPCC Service Coordinator provides support and supervision to Individual Support Service(ISS) staff working in the program on a consistent and regular basis to ensure the quality of services being provided. The CAPCC Service Coordinator monitors the supports provided by contracted service agencies and Care Companions hired by the individual or their guardian/family to ensure services are delivered based on the individual’s service plan and in accordance with the service agreement with the contracted agency.

1. Specific

**Responsibilities**:
a. Individual Support
- Establish a supportive and effective working relationship with each Individual that facilitates growth and opportunity for independence.
- Promote the Individual’s right to autonomy and normalcy.
- Identify each Individual’s interests, needs and skills to determine appropriate support.
- Develop a knowledge of resources and support services in the community
- Utilize community resources to enhance services to the Individual
- Promote the development and utilization of natural community support, where possible.
- Promote the Individual’s development of their support network
- Promote safe and healthy environments in the Individual’s home and in the community.
- Ensure appropriate support to each Individual, adhering to ethical standards outlined in policy and funding standards.
- Ensure all meetings/appointments are kept and documented.
- Promote the Individual’s health and well being.
- Act as a role model, displaying professional attitudes and behaviour.
- Facilitate educational/support opportunities for caregivers.

b. Staffing, Supervision and Service Monitoring
- Ensure a complete and thorough program orientation for each new Supports for Community Living (SCL) staff and each new organization contracted with.
- Provide consultation and input to the Program Coordinator in developing and managing the SCL staff master schedules
- Ensure SCL staff complete all mandatory training,
- Provide support and supervision to SCL staff on a consistent and regular basis.
- Provide support to contracted organizations to promote effective service delivery of the SCL supports.
- Monitor ongoing performance of SCL staff formally and informally.
- Monitor SCL supports delivered by contracted organizations to ensure effectiveness.
- Evaluate SCL staff performance in accordance with policy.
- Review position descriptions with SCL staff at annual evaluation.
- Ensure personnel files are kept up to date and ensure confidentiality at all times.
- Promote open communication at all times with both SCL staff and contracted organizations.
- Address SCL staff issues, differences and grievances as they arise in an open, supportive manner. Document and relay major problems and SCL staff grievances to the Program Coordinator.
