**RAI Back-Up Coordinator (Part-time)**

Copernicus Lodge is a not-for-profit, charitable and ethno-specific organization made possible through the efforts of the Polish Canadian community and the vision of Father Michael Smith, OMI. Located in the heart of the Roncesvalles community in the heart of west-end Toronto, we provide a continuum of care in a predominantly Polish, Roman Catholic environment to meet the changing needs of our seniors. The Lodge is comprised of an Adult Day Program, 200 independent, affordable self-care apartments with a Supportive Housing program, and a 228-bed Long Term Care home.

We are seeking a dynamic and innovative individual to step into our open Part-time Back-Up RAI Coordinator role in our Long Term Care Home, who will work enthusiastically in a client and family centered and comprehensive services environment that meets client needs. They will ensure resident safety and work within a quality improvement framework to identify opportunities for enhancing the quality of the care provided to our residents.

It is ideal for nurses looking to develop their expertise and explore other career paths. The RAI Back-Up Coordinator (Part-time) will be a champion in promoting excellent care while also demonstrating exceptional leadership, communication, problem solving, and conflict resolution skills.

**Job Summary**:

- Participate in RAI training
- Monitor and provide on-site support to all frontline RAI-MDS users in the Home
- Communicate regularly with the frontline RAI users and management staff
- Auditing of RAI-MDS, care planning and assessments
- Complete RAI-MDS assessments as required
- Strong knowledge of RAI-MDS
- Good attention to detail
- Strong organizational skills

**Qualifications/Skills**
- Successful completion of a Practical Nursing Diploma Program
- Member in Good standing of the College of Nurses of Ontario as a Registered Practical Nurse
- Post-secondary Certification or training in gerontology or related fields, is desirable
- Minimum one year experience as an RPN in Geriatrics.
- Demonstrated interest in the RAI Coordinator Role
- Current CPR Certification.
- Excellent interpersonal and communication skills - ability to speak fluently and read/write in English
- Speaking knowledge and understanding of the Polish language and customs is a definite asset. Failing this, a genuine interest and effort to learn basic Polish words/phrases to assist in providing resident care and services;
- Minimum one year experience as an RPN in Geriatrics
- Excellent physical assessment skills, critical thinking skills, and problem solving skills.
- Demonstrated understanding and commitment to resident and family-centered care.
- Demonstrated ability to organize resident care demands and to set priorities for care needs
- Demonstrated ability to engage and educate residents and families.
- Demonstrate commitment to quality improvement, ongoing professional development, and reflective practice
- Excellent computer skills and knowledge of electronic care planning and delivery.
- Demonstrated ability to work collaboratively within an interprofessional team as well as independently.
- Excellent attendance record and ability to maintain same.
- Able to work shifts/flexible hours including weekends.
