**Salary**: $45,000 - $48,000 per year

The YMCA Nova Scotia Works Employment Services Centres is looking for a dynamic individual and connector of people to join our team as Case Manager at our Halifax location.

**The Opportunity**

With funding from Employment Nova Scotia, the YMCA Nova Scotia Works Employment Services Centres, deliver a comprehensive service to assist individuals with finding employment. We provide one-on-one case management, employment assessments, needs determinations, case management, job development and workshops, as well as linking clients to appropriate employment opportunities and community resources.

As a Career Practitioner/ Case Manager you will:

- Administer and interpret needs assessments to clients seeking assisted services
- Assess client’s employment barriers and assets
- Develop a mutually agreed upon plan of action plan, taking the client from unemployment to finding and maintaining employment
- Accurately document action plans and interventions in client database ( LAMPSS)
- Provided ongoing follow up to ensure that an agreed-upon action plan is being followed and achieving expected outcomes
- Make appropriate referrals for specialized services to partnering organizations
- Submit monthly statistical reports that are congruent with client database

**Qualifications & Skills**
- Post-secondary education in Human Service field or related field of study or a combination of education and work-related experience
- Demonstrated experience in client services and facilitation
- Direct experience working with diverse groups
- Knowledge of community programs and resources
- Demonstrated ability to work with unemployed persons who face barriers to finding and maintaining employment
- Ability to work independently and as part of a team
- Strong interpersonal skills including tact, diplomacy, and ability to establish rapport
- Demonstrated ability to display sensitivity, managing expectations and diplomacy for internal and external contacts
- Excellent verbal, written communication skills, including presentation/ facilitation skills
- Valid Nova Scotia Driver’s License or access to reliable transportation

- Completion of YMCA’s Child Safety & Protection Orientation within first week of employment
- Completion of YMCA Child Safeguarding Training within probationary period
- Valid First Aid and CPR certificate if required for the role
