**Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families**:
The Lead, Health Equity and Community Engagement supports the efforts to reduce health disparities and achieve health equity by engaging with equity-seeking communities and address health equity issues.

The position builds strategic partnerships with both internal and external partners to develop and promote innovative mechanisms for service design and delivery. The position supports the integration of health equity lens and Equity, Diversity and Inclusion (EDI) principles in the health system through enhanced community engagement and tailored interventions. Responsible for the planning, implementation, maintenance, and evaluation of community engagement and partnership initiatives in order to build system capacity. The Lead, Health Equity and Community Engagement will play a critical role in helping to identify disparities in health care access, service provision, satisfaction, and outcomes.

**Responsibilities**:

- Initiates and manages projects that will support Fraser Health become a leader in health equity and community engagement by enhancing system capacity and building engagement skills, and tools.
- Leads the overall coordination of health equity related community initiatives across Fraser Health. Leads and fosters active community engagement in idea generation, planning, implementation, integration and evaluation.
- Develops a scalable, flexible and sustainable model for community engagement and partnerships. Develops mechanisms to ensure that community engagement is responsive to changes in diversity and to feedback from patients, and providers.
- Builds and oversees a community engagement model that enhances system capacity for targeted interventions, including supervision of volunteers as required.
- Identifies and develops collaborative partnerships with healthcare providers, professional organizations, not for profits, academic researchers, other levels of government, and community groups to promote community partnership initiatives, increase capacity for innovation, encourage community engagement in service design and delivery, and create awareness through education.
- In consultation with Corporate Communications develops and implements a multi-level communications strategy for diverse community engagement including internal communications and collaboration with other key partners on the development inclusive communication strategy.
- In consultation with partners, develops and/or contributes to a community engagement programs to better understand the patient, provider, setting and systems factors that influence implementation of best practice in community engagement and partnerships.
