This position is open to all qualified OPSEU staff throughout Lakeridge Health commencing June 27, 2023 and closing at 11:59 pm on July 5, 2023.

Pinewood Centre provides a range of services for issues related to substance use, gambling and concurrent disorders and assists youth, adults and family members in Durham Region. This position will function as part of the Community Treatment/Opiate Team and provide comprehensive, intensive brief case management and treatment to clients who use opioids and have been treated in a Lakeridge emergency department or discharged from inpatient services. A key function will include assisting these individuals to navigate the broader community system of care to access services. The Addiction Counsellor will work closely with Durham Hospital to Home program, Emergency Departments (ED) and the Rapid Access Addiction Medicine (RAAM) clinics to provide a continuum of evidence-based, client-centred care. These functions include: initial screening, consultation, and intake services, assessment, referral, linking, advocacy, integration of required services, initial treatment planning and provision of services and collaboration with other community-based service providers.

DUTIES AND RESPONSIBILITIES:

- Maintain a professional collaborative relationship with all programs within the Mental Health and Pinewood Program, the ED, Primary Care, needle exchange programs, CMHA, DMHS and other agencies in the community;
- Proactively work on behalf of the system to identify the appropriate resources for each client and to assist clients and families to navigate their way through the complexities of the health care and social support systems;
- In collaboration with the client and other community supports, create a client service agreement which defines the treatment plan, the crisis response plan, identifies the parameters of each services and alternatives to the Emergency Department and lists the professionals and their agencies within the circle of care;
- To provide information and education to programs with Lakeridge Health and in the community as required regarding addiction issues and the range of services and supports across the continuum of care in keeping with an ant-stigma approach to treatment;
- Review client assessment information and prepare individualized client care treatment plans outlining strengths and goals that address clients' needs for long-term stability and growth;
- Ensure that clients' basic needs of food, clothing and shelter are met;
- Provide supportive services assisting clients with real-time appointments, through such activities as: arranging transportation to appointments and/or accompanying clients to appointments, as necessary, to ensure linkage to services;
- Provide assistance to clients with establishing peer and family support networks; and minimize risk by adhering to Pinewood Centre's policies and procedures and by identifying, assessing, evaluating, and controlling risks to ensure the well-being of staff and clients.

QUALIFICATIONS:

- University degree
- Have and maintain a valid driver's licence; have and maintain consistent use of reliable transportation and appropriate operators insurance;
- Demonstrated experience with substance use populations, particularly those experiencing opioid dependence;
- Demonstrated experience in the provision of screening, consultation, and intake services; case management functions (assessment, referral/linking, advocacy, monitoring, and treatment planning) and family supportive counselling;
- Experience with telephone counselling required;
- Exceptional interpersonal, communication and organizational skills;
- Excellent working knowledge of theories of addiction, evidence-based treatment models and best practice approaches;
- Knowledge of other key issues associated with substance use, opiate replacement therapies, harm reduction, concurrent disorders, crisis intervention, physical health issues, homelessness, family issues, and women's issues; demonstrated ability to work closely with the community and to collaborate and network;
- Ability to recognize and diffuse escalating or potentially violent clients;
- Ability to work closely with a clinical team, and independently in a manner consistent with high professional standards;
- Experience with program and service development;
- Knowledge of community resources and the provincial addiction system;
- Commitment to a client-focused philosophy of care;
- Excellent record keeping and file management;
- Willingness to be clinically supervised and pursue ongoing professional development;
- Must provide a recent Criminal Investigation Reference (for persons involved with the vulnerable sector)
