Bariatric Patient Coordinator - Clinic 360 (Bariatric and Plastic Surgery Clinic)

Job Summary:
Reports to: Bariatric Director, Nursing Director

**Responsibilities**:

- Serve as a clinic ambassador for the 360 Sleeve program, representing Clinic 360 to potential patients.
- Act as a proactive and autonomous lead in coordinating and providing continuity of patient care.
- Facilitate lab tests and results for patients and maintain efficient communication.
- Respond to patient inquiries, providing education, information, and a caring approach to patients.
- Screen patients following consultation appointments, ensuring they meet the necessary criteria for bariatric surgery. Work collaboratively with other departments (ie: Nutrional Services & Anesthesia) to facilitate the patients consultation appointments in preparation for surgery.
- Maintain patient files and ensure surgeons and the team have the latest information on upcoming surgeries and procedures, ensuring that all required documentation is properly prepared and in order for surgery.
- Assist surgeons during consultations and schedule follow-up appointments as directed by the physician.
- Collaborate with the marketing team, sharing patient insights, and acting as a thought leader for campaigns.
- Maintain a strong online presence to effectively engage with patients.
- Coordinate care with other clinic staff to maintain continuity of care and deliver exceptional service to patients.
- Address patient complaints and concerns, escalating them to the Medical or Nursing Director as needed.
- Collaborate in the completion of required documentation, including consent forms.

**Requirements**:

- Bachelor's degree in a relevant field.
- Minimum of 1 year of relevant experience in patient care coordination, medical office administration, or similar roles.
- Nursing background is considered an asset.
- Experience with surgical, bariatric, or weight loss patients.
- Superior verbal and written communication skills.
- Proficiency in MS Outlook and Word.
- Experience with electronic medical record systems, and knowledge of Telus CHR is considered an asset.
- Strong customer service orientation with a caring and personable demeanor.
- Ability to effectively present and communicate product information to potential customers.
- Highly motivated, proactive, and autonomous with a passion for achieving sales success.
- Ability to work in a fast-paced, target-driven environment.
- Strong organizational skills.
- Flexibility to adapt to changing priorities and business needs.
