**Job Description**:
In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Supervisor, the Case Coordinator, Brain Injury Program, in consultation with the client, family and interdisciplinary team members, provides case coordination to initiate and develop a plan of care, counsel clients and families on issues that have impact on their care, create referrals to other community resources, and evaluate care plans. The Case Coordinator identifies and develops best practice to optimize delivery that aligns with Island Health's Brain Injury Program's philosophy to provide and coordinate effective, individualized assessments and treatments for clients with complex neurological, traumatic and rehabilitative conditions into a practical continuum of care.

Within a defined geographic area and established guidelines, the Case Coordinator negotiates, develops, implements and oversees service delivery contracts. This includes evaluating performance, assessing utilization, maximizing service delivery and assuring contract compliance..

Travel is a regular requirement of this job. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority in accordance with the service assignment and may require the use of a personal vehicle.

**QUALIFICATIONS**:
**Education, Training And Experience**:
Bachelors' in Allied Healthcare Discipline. One (1) year recent related experience or an equivalent combination of education, training, and experience. Experience working with diverse populations such as families in crisis or after trauma. Community based experience working with adults with acquired brain injuries.

Valid BC Driver's License.

**Skills And Abilities**:

- Demonstrated knowledge of community development and community issues related to brain injury services.
- Demonstrated ability to plan and implement plans of care for clients with acquired brain injuries.
- Ability to work independently with mínimal supervision.
- Ability to assess, plan and coordinate effectively as a member of an interdisciplinary team.
- Demonstrated problem solving skills.
- Demonstrated ability to counsel and teach clients and their families based on their priorities.
- Ability to prioritize and organize workload.
- Excellent communication (verbal and written).
- Demonstrated ability to deal with conflict situations.
- Physical ability to perform the duties of the position.
