Dementia Care Facilitator 12 Month Contract

Full Time Position Salary Range: ( $33,378.00 - $47,117.00 )

Salary to be commensurate with experience.

Location: Brunel Day Program (BRNL)

**POSITION PURPOSE**

This entry level position focuses on developing the necessary skills to support Persons Living with Dementia (PLWD) and their Care Partners. Through training and hands-on experience in our adult day programs, you will develop the skills necessary to successfully support individuals who are living with Alzheimer disease or related dementia (ADRD) and their Care Partners (CP’s).

**WHAT YOU GET TO DO**
- Be creative in developing, implementing & evaluating therapeutic recreation programs for PLWD.
- Report on Client goals, and progress in achieving these goals.
- Engage and build relationships with PLWD and their CP’s.
- Assist PLWD with activities of daily living.
- Support the Client Service Team by assisting at different sites as needed to maintain the safety and wellbeing of all clients attending the day programs.
- Participation on an ASP committee or working group of your choice.

**WHAT YOU ALREADY HAVE**
- Degree, Diploma or certificate in Therapeutic Recreation, Activation, Social Services, Gerontology, or Personal Support Worker.
- Understanding of Alzheimer’s disease and related dementias.
- Strong understanding of client-centered philosophy.
- Competency in Google Workplace and MS Office tools.
- Any additional languages would be considered an asset.
