Alzheimer Society Peel (ASP) was chosen as the lead agency for the Behavioral Supports Ontario (BSO) initiative which provides behavioural health care services for older adults in Mississauga, Halton, and South Etobicoke with, or at risk of, responsive behaviours associated with dementia, complex mental health, substance use and/or other neurological conditions across all sectors.

The BSO LTC Stabilization Facilitator (BSO LTC SF) is a part of the BSO Mobile team and works in collaboration with BSO LTC embedded teams across the region to ensure that the resident and staff are supported during the transition into LTC to manage the behaviours and improve quality of life for the resident.

The BSO LTC SF is responsible for providing short-term, person-centered transition support to residents during their move into LTC in the Mississauga Halton Region. The role will collaborate with the Community BSO team and the LTC team to ensure a successful and sustainable move into LTC through the implementation of an individualized transition plan.

The BSO LTC SF will also provide additional support to the embedded LTC BSO team to develop a plan of care for residents experiencing responsive behaviors, to reduce risk and improve resident quality of life. The BSO LTC SF will collaborate with the interdisciplinary team including other service providers and system partners, to improve the care for older adults and their care partners.

**Key Accountabilities**:

- Understand and commit to the goals, framework, and principles of Behavioural Supports Ontario (BSO).
- Works collaboratively with the BSO embedded team to develop a person-centered, individualized behaviour management plan which could include triggers, interventions, and communication strategies.
- The BSO LTC SF will trial interventions while also utilizing behaviour tracker tools (BSO-DOS, ABC Charting), observation, staff huddles, and staff documentation to monitor ongoing effectiveness of recommendations.
- Assist with activities of daily living in collaboration with the BSO Team and LTC staff. Care responsibilities may include but are not limited to: personal care, fluid and nutrition intake and monitoring, positioning and ambulation, and ensuring the safety and comfort needs of referred residents.
- Work collaboratively and build rapport with key partners such as LTCHs, Seniors Mental Health Outreach Team, community support programs and other BSO team members such as embedded BSO team, Community Support Worker (CSW), Psychogeriatric Resource Consultant (PRC), mobile team RN to ensure holistic support to LTC residents settle into the LTC environment.
- Provide resident-specific information and education to facilitate an understanding of the referred resident’s responsive behaviours, triggers, and interventions.
- Strengthen staff's capacity to manage responsive behaviors through modeling, coaching, and mentoring.
- Participate in care conferences, team huddles, BSO meetings, education and/or training as required.
- Identify the need for appropriate referrals to other BSO team members or Alzheimer Society Peel services to support the resident, care partners and staff.
- Navigate through the LTC documentation system to gather appropriate background information to support the referral (Point Click Care, Activity Pro).
- Responsible for maintaining accurate and complete records of care and ensuring documentation is completed in a timely manner
- Maintain accurate and timely monthly statistics through the identified Statistical Reporting System. -Requires flexible hours including some evenings and weekends based on resident needs.

**Education and Experience**:

- College/University Degree in a relevant health or social science field; training in Gerontology or equivalent
- 3-5 years of experience working with the elderly with Alzheimer's Disease and other related dementias preferred.
- Behavioral support experience in LTC is considered an asset
- Working knowledge of various regulatory requirements within the Health Services environment.
- Experience working in LTC with dementia care training (e.g., U-First, P.I.E.C.E.S, GPA etc.) is considered asset
- CPR & First Aid Certificates
- Intermediate to Advanced - Google Workspace,Word, Excel, PowerPoint & Outlook
- Moderate experience with EHR, data tracking, database documentation, logging data entry
- Strong written and oral communication skills
- Self-directed, team player
- Valid driver's license and access to a reliable car
