**BSO ER Behaviour Support Counselor (“ER BSC”)**

**Key Accountabilities**:
**Emergency Room Support**
- Assess the needs of patients with dementia and BPSD upon arrival in the emergency room, focusing on creating a supportive and calming environment.
- Provide immediate emotional support and crisis intervention to families and caregivers.
- Collaborate with ER staff to ensure the safety and comfort of patients with dementia, offering behavioral strategies to manage symptoms.
- Assist in developing and implementing personalized behavior management plans while in the ER.

**Family and Caregiver Support**
- Educate families about dementia, BPSD, and effective strategies for handling challenging behaviors.
- Facilitate referrals and assist in the discharge process for individuals presenting comorbidities such as mental health, substance use, and/or other neurological conditions
- Offer reassurance, guidance, and coping strategies to families in how to manage high-risk or complex situations.
- Facilitate communication between families and healthcare professionals to ensure a clear understanding of care plans.
- Ensures patients and caregivers are provided with timely and appropriate information and referral to the Society and BSO Services where appropriate
- Identifies staff and caregivers who would benefit from additional education and makes the appropriate referrals and internal escalation as needed
- Collaborates with family members and caregivers, offering support, information, and practical strategies for managing their loved one’s symptoms

**Community Navigation and Follow Up**
- Conduct follow-up visits in the community to assess the ongoing needs of patients and their families.
- Work with families to develop and refine behavior management plans, providing continued education and support to mitigate risk
- Help families navigate healthcare and social services. Connect families with community resources, such as support groups, respite care, and specialized dementia services.
- Monitor and document progress, adjusting behavioural care plans as necessary.
- Guide patients and families in navigating complex healthcare and social services including referrals to specialists, home care, and community organizations

**Collaboration and Advocacy**:

- Collaborate with a multidisciplinary team, including physicians, nurses, social workers, and community organizations, to provide comprehensive care.
- Advocate for the needs of patients with dementia and their families, promoting person-centered approaches to care.
- Establish and maintain relationships with local dementia support services, healthcare providers, and community organizations to ensure integrated and comprehensive care.

**Documentation and Reporting**:

- Maintain accurate and confidential records of interactions with patients and families.
- Report on outcomes and identify areas for improvement in the behavioral support services provided.

**Qualifications and Skills**:

- An undergraduate degree in a related health discipline with strong knowledge in geriatrics is required (e.g. Nursing, Social Work, Psychology, Occupational Therapy). At a minimum, a diploma in Social Service Work, Recreational Therapy, Gerontology, or a related field will be considered in combination with demonstrated work experience in the field of geriatrics
- Training and experience in dementia care, BPSD management, crisis intervention, or behavioral support.
- Knowledge and/or experience supporting individuals with mental health concerns, substance use, and/or other neurological conditions will be considered an asset
- Strong understanding of dementia and its impact on patients and families.
- Minimum 3 years of work-related experience; Experience working in a healthcare or acute care setting is an asset*
- Minimum 3-5 years of experience working with the elderly with Alzheimer's Disease and other related dementias preferred
- Working knowledge of various regulatory requirements within the Health Services environment
- Behavioural intervention experience
- Excellent communication, active listening, and problem-solving skills.
- Ability to work collaboratively with healthcare teams and community resources.
- A compassionate and empathetic approach to supporting individuals and families in distress.
- U-First, P.I.E.C.E.S, GPA Certification is an asset
- CPR & First Aid Certificates -Intermediate to Advanced - Word, Excel, PowerPoint & Outlook -Self-directed, team player -Valid driver's license and access to a reliable car
