**Job Summary**:As a member of the Client Services team, the Community Access Coordinator is responsible for assisting individuals between the ages of 18 and 65 residing in long care facilities with physical, mental, emotional, cognitive, sensory or social limitations that may affect their attitudes, ability and motivation to participate in leisure and maintain healthy, balanced lifestyles.

The Community Access Coordinator may work in collaboration and develop/maintain working relationships with the CAPCC Participant, Director of Care, Nurses, Unit Managers, Unit Clerk’s, Client Service Managers, Recreational Therapists, Social Workers, Private and Public Guardians and Trustees, and family members to develop individualized service plans for participants, that will increase access to activities in the community in order to improve quality of life through increased social interaction and reduced isolation.

**Key Areas of Responsibility**:

- Work with individuals, their informal supports and community partners to identify and develop individual and community supports to facilitate community participation
- Work with individuals, their informal supports and community service and support partners to develop individualized plans identifying functional needs and appropriate service and support options
- Establish and maintain an understanding of and working relationships with community programs and services, both generic and disability specific
- Develop working relationships with Long Term Care facilities and staff within those facilities
- Submit monthly invoices and reports
- Assist clients with exploring lifestyle, recreation and leisure options
- Educate support staff and community based programs as well as support clients to make use of community services related to their recreational, medical, learning, and leisure needs
- Identify programs that provide relevant services for clients to access those services
- Assist clients in the development of support networks and provide support, training and guidance to individuals and their families
- Encourage independence and self-confidence and motivate clients and facilitate adjustment to change
- Participate in case conferences, case management sessions, and in Regional Advisory Committee meetings
- Advertise program
- Source out and interview potential care provider organizations. Also, assist clients in interviewing, hiring, supervising, training and terminating staff and volunteers who provide care to the clients
- Remove barriers that prevent clients from participating in meaningful leisure
- Educate clients about the physical, mental, social and emotional health benefits of participating in recreation/leisure activities
- Assess client skills, needs, interests and values related to recreation and leisure
- Develop and deliver service plans and programs
- Keep notes and maintain records about client participation and communicate with the team
- Evaluate the success of service plans and programs

**Characteristics, Skill Sets, Experience and Education**:

- Post-secondary education in a human services related discipline such as social work, rehabilitation or recreation therapy
- Working knowledge of the continuing care system and local community resources and services
- Cross-disability knowledge and experience in cross-disability case management and service coordination
- Ability to work independently and as part of a team with other professionals.
- Excellent organizational, interpersonal, communication, decision making, coaching and problem solving skills
- Enthusiasm, a positive attitude, persistence and the ability to motivate and inspire others
- Creative ability for developing new approaches and programs
- Ability to establish rapport with people of all ages and backgrounds
