What will you do?

Home and Community Care Support Services is looking for a dynamic individual to manage the development, implementation, and continuous quality improvement of assigned Home and Community Care service initiatives and programs. Reporting to the Director, Business Analytics & Program Development, this position will support efficient and effective services provided to patients and their families in hospitals and in the community. Dual reporting may occur depending on the client service department priorities and initiatives assigned. The Manager, Patient Services provides leadership, direction and operational oversight to ensure high quality patient services are provided.

KEY ACCOUNTABILITIES:

- Regional Home and Community Care Client Services Program Development;
- Leads the development of effective education and training to support program implementation and compliance;
- Collaborates with external partners such as Community Support Service Agencies in program development and management of programs that impact Patient Services delivery (i.e. Assisted Living, Low Acuity Personal Support);
- Coordinates with Project Management as required to support Program Development initiatives;
- Conducts routine program monitoring and evaluation and provides feedback to Patient Services management regarding trends, compliance and areas for improvement;
- Researches, analyzes, and develops Patient Services improvement initiatives based on findings from program monitoring;
- Ensures patient service programs are aligned with HCCSS policies, relevant legislation and professional standards;
- Assists in the development of policies and procedures and ensures that the programs meet both organizational needs and relevant professional standards;
- Utilizes change management principles to support program development and implementation;
- Performs other duties as assigned.

POSITION REQUIREMENTS:

- Post-Secondary degree in a health-care related field, business administration or relevant field;
- Minimum of 5 years of experience in a healthcare environment with demonstrated leadership experience;
- Considerable experience in program development and/or project implementation;
- Significant experience in change management;
- Effective communication and interpersonal skills;
- Working knowledge of external community support services (i.e. Assisted Living)
- Ability to plan, execute, and improve work processes to ensure objectives and deliverables are met;
- Excellent organizational skills, ability to successfully prioritize, and manage multiple projects simultaneously;
- Strong coaching, negotiation and conflict resolution skills in order to find resolutions regarding program development and interpersonal issues among employees;
- Good knowledge of HCCSS priorities, policies, practices, and service standards;
- Considerable hands on knowledge of MS Office including Outlook, Access, Excel, PowerPoint, and Word;
- Advanced oral and written proficiency in English is essential, with proficiency in French considered an asset.
