**_ Program Overview - Crisis Outreach Service for Seniors, Community Care_**
- This position works on behalf of WoodGreen in an inter-agency, multi-disciplinary Crisis Outreach Service for Seniors (COSS) serving seniors with physical, cognitive, mental health, and other concerns causing them to be in crisis. This service is offered through a partnership between WoodGreen, Haven Toronto, South Riverdale Community Health Centre, Reconnect Community Health Services, and LOFT Community Services with the goal of ensuring a continuum of care for seniors with cognitive, mental health, and substance use issues._

What You Will Do_**
- _
Participate in crisis shifts; going out to visit clients in their homes or other community locations with a partner._
- _ Assist with providing preliminary assessment of risk in crisis situations_
- _ Assess client needs, develop, and implement service plans with short and long-term objectives._
- _ Provide short-term intensive case management services to a caseload of seniors with mental health and substance use issues, including ongoing assessment, supportive counseling, service navigation and care coordination._
- _ Assist with providing crisis intervention and practical assistance to clients and/or family or caregivers as required in homes and community settings._
- _ Assist with providing crisis stabilization and short-term case management services to clients through the development and implementation of a care plan to address their priority and emerging needs. _
- _Assist clients to access acute care and emergency services when appropriate and help facilitate client discharges from local emergency departments._
- _ Assist with monitoring clients, identifying additional needs, and reactivating supports. _
- _Assist in liaising with convalescent care, transitional care, acute care, emergency departments, rehabilitation centres, and complex continuing care to ensure that clients who have been hospitalized have appropriate follow-up services._
- _ Refer clients and caregivers to a range of community resources, including meals, drop-in centres, social recreational programs, clothing exchanges, etc._
- _ Promote the healthy functioning of the team and unit and participate in meetings regularly._
- _ Participate in ongoing training and education._
- _ Participate in regular supervisory sessions and performance appraisals._
- _ Maintain appropriate records and documentation in a timely manner and collect statistical data required for the accountability of the program._
- _ Work flexible hours based on the needs of the clients, their caregiver, and the service._
- _ Support team members/students and participate in team meetings._
- _ Ensure that anti-oppression, community development, and harm reduction principles are achieved in the program._

What You Bring to the Team_**
- _
BSW or a related undergraduate degree._
- _ Registration in good standing with the Ontario College of Social Workers and Social Service Workers or College of Registered Psychotherapists of Ontario or an equivalent._
- _ 2+ years’ experience in crisis support, intervention, conflict mediation, and supporting seniors with mental health and substance use issues from a recovery and harm reduction approach._
- _ Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption)._

- _ Knowledge of issues affecting older adults, geriatric populations, the health sector, community resources, and related services._
- _ Demonstrated ability to follow policies, procedures, and adhere to legislation._
- _ Understanding of clinical practices and care models.
