Become a key community connector to support building a collaborative and comprehensive Primary Care Network in qathet. We are seeking an outgoing Primary Care Services and Attachment Coordinator to support networking and promotion of all available health and wellness services within the qathet region. The qathet Primary Care Network is currently in start-up following years of planning and engagement across partners including the qathet Division of Family Practice, Vancouver Coastal Health Authority, the Tla'amin Nation, and other community partners. Join our collaborative team to support a coordinated and comprehensive primary care system in qathet.

**ROLE SUMMARY**

**KEY RESPONSIBILITIES**
- Maintain current knowledge and relations with community agencies and service providers to keep up to date on changes in their programming and services.
- Support updating resource directory websites by providing information about available qathet services (e.g., BC Pathways Directory and FETCH).
- Assist primary care providers by providing information on local community services and assists them in finding patient resources.
- Liaise with family physicians and nurse practitioners to determine ongoing capacity for accepting new patients.
- Coordinate patient referrals from multiple sources, including HealthConnect BC, to primary care providers accepting patients, while considering urgency, patient need, and provider capacity.
- Oversee public communication on the availability of primary care services, urgent care pathways, and the HealthConnect BC registration process.
- Support and promote the ongoing use of HealthConnect BC to maintain an accurate waitlist of patients seeking a primary care provider.
- Liaise with the HealthConnect BC team to ensure patient registration process aligns with the local attachment process.
- Follow up with family physicians and nurse practitioners after patient attachment to ensure the attachment has been successful.
- Update and maintain data on patient attachments and generate standard reports for the Division and Ministry of Health, ensuring that data is maintained according to privacy laws.
- Support Primary Care Networks (PCNs) in developing clinical programs to improve patient care.
- Ensure cultural safety and humility in all aspects of support provided by the Attachment Coordinator.

**QUALIFICATIONS**
- Preferred Education, Training and Experience_
- Post-secondary diploma, certificate, or degree and minimum of three years of experience working in a healthcare setting in an administrative and/or other capacities.
- Previous experience working with physicians is an asset.
- Previous customer service training/experience is an asset.
- An equivalent combination of education, experience and expertise may be considered.
- Preferred Skills and Abilities_
- Knowledge of BC health care system and primary care.
- Excellent customer service and communication skills; ability to communicate effectively, both verbally and in writing; and ability to engage physicians and clinic staff.
- Excellent interpersonal and relationship-building skills with proven diplomacy and ability to liaise with physicians and patients in a professional and courteous manner.
- Exceptional time management skills with the ability to handle multiple tasks simultaneously.
- Excellent collaboration and coordination skills.
- Proven ability to use Microsoft Office Suite tools for collecting and presenting information, e.g., Word, PowerPoint, Excel.
- Strong and adaptable technology skills, with an understanding of health care and patient-centered technology preferred.
- Knowledge of electronic medical records (EMRs) is an asset.
- Attention to detail, accuracy, and thoroughness when maintaining and summarizing information, data, and records.
- Ability to work independently and in cooperation with others.
- Proven ability to plan, lead, coordinate and inspire positive change.
- Group facilitation skills are an asset.
- This role may evolve; flexibility and adaptability to change are required.

**WORK TERMS AND HOURS**
- We anticipate the position to average 30 hours per week. This position is an evolving role with a need for some flexibility as demands will fluctuate from week to week and include some evening and early morning engagements.

**TOTAL COMPENSATION PACKAGE**
- This position pays between $30-$35 per hour, commensurate with qualifications and experience.
- Travel and expenses are reimbursed.
